1 Better Health Partnership Conference Topic: Accelerating Health Information Exchange Description: The promise of timely and seamless exchange of patient information across health care systems remains unfulfilled. In Greater Columbus, an initiative is getting underway to speed coordination among health care systems and social service agencies in the medical neighborhood with an electronic path for referrals.

Identify actionable approaches for implementing shared referral infrastructure in the medical neighborhood.

We serve as a catalyst, convener, and coordinator of healthcare transformation & learning in Greater Columbus. Our Collaborative Approach The status of our healthcare situation is not acceptable in terms of its prevention, quality, safety, transparency, or cost. We strive for win-win solutions recognizing that change is required by all consumers, providers, and purchasers. We believe the best solutions come from a collaborative approach we seek no authority in our role as a catalyst/convener/coordinator We work together with our public-private partners to accelerate and scale innovation.

In January 2014, UWCO and HCGC convened funded member agencies to determine the challenges organizations face when sending and receiving referrals, and identify the healthcare providers they most frequently exchange referrals with. 13 2015 Project Overview WHAT: Spread best practices to improve coordination of care between providers and social services in Greater Columbus. HOW: Implement cloud-based, shared referral infrastructure among local healthcare providers, social service agencies, and other healthcare stakeholders Objectives Convene interested social service agencies and healthcare providers to plan for implementation of initial phase of project. At least ten social service agencies and five local healthcare providers participating in project implementing CliniSync to send and receive referrals. Collect project measures, document lessons learned, and share learning with public-private partners
