3 Authorization Statement Divine Medical Billing, Inc is authorized by the Tennessee Higher Education Commission. This authorization must be renewed each year and is based on an evaluation by minimum standards concerning quality of education, ethical business practices, health and safety, and fiscal responsibility. History and Ownership Divine Medical Billing was founded in August of 2005 by Mylikia Ross Franklin. The company was founded to fill a need for competent, skilled, proficient, and capable medical billers. Divine Medical Billing's, billing service grew rapidly and went from severing providers in just Tennessee to serving providers in more than 19 states. 5 Facilities Divine Medical Billing, Inc is located at 306 South Church Street, Suite B, in downtown Murfreesboro, TN near the square. This school is centrally located, with access to the Murfreesboro Public Library, city and county buildings, food, and shopping. The school is housed in a 2,100 square foot professional office space. This facility includes a classroom, offices, and lounge area. We utilize computer, projectors, and white boards for educating and instructing. Institutional Purpose and Objectives Divine Medical Billing, Inc institutional purpose is to education and prepare professional medical billing specialist. Our objective is to provide the training necessary to be a priority contributor in any provider s office, ancillary office, or medical billing service. Students completing our program will have an in-depth understating of the entire medical billing process from credentialing to collections. Enrollment Procedures Prior to enrollment the following requirement must be met. Tour the school. Submission of a completed application and the application fee of $100. Student must submit proof of graduation from high school ore GED within 30 days from the start date of the program. Student must have paid for their book fee of $875. Student must pass a criminal background check. Student must pass a drug test. Page 4

6 Attendance Policy Students who miss 20% of scheduled contact hours may be withdrawn from the class. Students may submit an appeal in writing if absence was due to extenuating circumstances. For example, illness, military duty, death of a family member, court appearance or jury duty. Tardiness will constitute a partial absence, 3 partial absences will equate to a full absence. Failure to report to 5 consecutive days will be cause for dismissal. Students may be readmitted to the program at the start date of the next upcoming class. It is important for the student to contact the school is he/she will be absent. Students will be responsible for make-up work and contact hours. Satisfactory Progress Standards To continue enrollment, students must maintain satisfactory progress. To achieve satisfactory progress toward diploma, students must maintain a certain grade point average and strive at a pace to complete program in the specified time frame. An overall average of 2.0 (C) is required to maintain satisfactory progress. Satisfactory progress will be measured at the end of each class. If a student cumulative grade point (CGPA) average falls below 2.0, the student will be placed on academic probation for the following class. At the end of the class, the students CGPA will be recalculated. If the students CGPA is 2.0 or above they will return to good academic standing. However, if the student's CGPA is still below 2.0, the student will be required to retake classes where they scored a less than satisfactory grade. Students will receive progress reports at the half way make through each class and upon completion. Page 5

7 Student Conduct All students are expected to conduct themselves in a professional manner. Student's appearance should be such that is appropriate for a business casual environment. Students are required to conduct themselves in an orderly manner while in school and should respect school property, faculty, and other students. Students who deface or damage school property or purpose or due to extreme carelessness will be subject to disciplinary action and may be held financially liable. While off school property, students are expected to conduct themselves in a favorable manner as to reflect positively on our school. Student's behavior in public can have a direct reflection on our entire student body. Students who fail to maintain these standards may be subject to disciplinary action and in extreme cases, subject to suspension or termination. Students who are charges with certain allegations pertaining to indentify thief and fraud will be suspended until court proceeding have completed. Students are required to report such charges within 24 business hours. If the student is found not guilty or if charges are dropped, the student will be eligible to return school. Divine Medical Billing is a smoke fee environment. However, smoking is permitted in designated outdoor areas. Food and drinks will be allowed in break areas only. Page 6

8 Program Program Objective Medical Billing Specialist Diploma Program 84 Weeks (excluding breaks) $6,750 Tuition Cost The Medical Billing Specialist program is designed for professional minded individual who would like to advance their careers in the medical billing field. This program takes an in-depth learning approach to the entire medical billing process. Students who complete this program will have advanced knowledge to manage claims processing and accounts receivables. Students will build knowledge of the following skills. Insurance Payers Terminology and Anatomy CPT and ICD Coding Credentialing and Contracting Claims Processing Insurance Payments Insurance Accounts Receivable Management Patient Billing and Collections Graduates are qualified for entry level employment opportunities in private practices, billing services, hospitals, assisted living facilities, and ancillary service locations. 9 Course Descriptions MBS100* Introduction to Medical Billing 32 Contact Hours Students will be introduced to the world of medical billing. They will learn the responsibility and duties of a Medical Billing Specialist. Students will learn about the Health Insurance Portability and Accountability Act (HIPAA). Students will examine the different roles that affect the daily duties of a Medical Billing Specialist, from the front office administration staff to the healthcare providers. Students will also be exposed to basic computer training to better utilize practice management systems, electronic health records, , instant messaging, word processing and spread sheets. MBS200* Healthcare Payers 32 Contact Hours In this course, students will the learn the various types of healthcare payers. Types of insurance plans will be studied, such as, Commercial, HMO, PPO, and Health Savings Account. Students will gain knowledge of Medicare, Medicaid, Tricare and Champva. This course will also examine Workers Compensation and Disability insurance. MBS300* Anatomy & Terminology 64 Contact Hours Students will learn the different functions and components of the human body. Students will review the following systems, Endocrine, Skeletal, Muscular, Circulatory, Lymphatic, Respiratory, Digestive, and Nervous, to name a few. In addition, students will also learn medical terminology. They will learn prefix, suffix, and root word parts. Students will also study word origin. MBS400* Medical Coding 80 Contact Hours Students will learn the practice of coding diagnosis, evaluation and management services, procedures, injections, and supplies. Students will learn how to use the ICD, CPT, and HCPS coding books. Students will also learn how to appropriately use modifiers. Students will learn to reference Correct Coding Initiative (CCI) edits resources for proper coding. Page 8

10 MBS500* Credentialing & Contracting 32 Contact Hours This course will review how to credential healthcare providers with various insurance payers. Students will learn about Medicare credentialing applications, CMS855I, CMS855R, CMS460, CMS588, and CMS855B. Students will learn about the contracting process and how to interact with provider representatives. MBS600* Claims Billing 80 Contact Hours Students will learn how to bill electronic claims and examine Electronic Data Interchange (EDI) functions. Student will also become familiar with CMS 1500 and UB04 forms. Students will learn how to bill secondary claims and manage clearinghouse connections/payer agreements. MBS700* Reimbursements 64 Contact Hours Students will learn how to read remittance advices and post payment from insurance payers. Students will also learn how to read and interpret denial reasons. This course will focus on basic math functions such as, adding, subtracting, algebraic equations, and finding percentages. MBS800* Insurance AR 64 Contact Hours This course will outline the methods used to manage account receivables (AR) and outstanding claims. Students will learn how to status and research claims with the various insurance payers, read AR reports, submit appeals, work denials and resubmit claims. MBS900* Patient Billing & Collections 32 Contact Hours Students will learn to submit patient billing and how to answer patient billing questions. This course will define the patient billing cycle and aging buckets. Students will learn collection rules and techniques to manage patient AR. Page 9

11 MBS1000* Career Readiness 192 Contact Hours This course will combine all the information taught in the Medical Billing Specialist program. Students will have the opportunity to put the learned knowledge to use in our Medical Billing Service. Students will work hands on with actual claims and data. Students will create resumes and practice job interviewing skills to prepare for entry to the medical billing workforce. Students will also prepare to sit for the Medical Billing Specialist exam. 12 Placement Assistance None at this time. Refund and Cancellation Policy Divine Medical Billing, Inc complies with the laws and regulation of the local, state, and federal government concerning cancellations and refunds. Official withdrawal must be made in writing and delivered to the school address by mail or in person. The official termination date for withdrawals and expulsions will be the day the student last attended class. Non-attendance does not constitute an official withdrawal. Refunds will be calculated based on the unused teaching hours at the hourly rate of $ If there is a refund due, the refund will be made to the appropriate account, lender, agency, or student. Students who withdraw, drop out, or are expelled, before the first day of class, or failing to begin class, will be refunded 100% of tuition, less administrative fee of one hundred dollars ($100.00), application and book fees. Students who withdraw, drop out, or are expelled, after classes have commenced and before expiration of ten percent (10%) of the period of enrollment for which he or she was charged, will be due a refund equal to seventy-five percent (75%) of tuition, less administrative fee of one hundred dollars ($100.00), application and book fees. Students who withdraw, drop out, or are expelled, after the expiration of the ten percent (10%) of enrollment and before expiration of twenty-five percent (25%) of the period of enrollment for which he or she was charged, will be due a refund equal to twenty-five percent (25%) of tuition, less administrative fee of one hundred dollars ($100.00), application and book fees. Students who withdraw, drop out, or are expelled, after the expiration of the twenty-five percent (25%) the student may be deemed obligated for one hundred (100%) of tuition, and fees. Page 11

13 Students who are unable to complete classes because the class has been discontinued by Divine Medical Billing, Inc the student will be refunded the sum of all amounts paid by student for such class. Application and book fees are nonrefundable. Student Grievance Procedure A grievance is a claim or expressed concern made by the student in regards to their educational experience. Students should initially discuss the grievance with instructor. If no resolution is obtain the student should make a written complaint and mail or deliver to the following. Students will be required to take each course outlined by Divine Medical Billing, Inc. Transfer of credit to another school is controlled by the receiving institution.
