2 Narrator: As of December 1, 2014, service providers who submit OCF-21 forms through Health Claims for Auto Insurance (HCAI) must be licensed with the Financial Services Commission of Ontario (FSCO) to invoice and receive direct payment from automobile insurers for specific "listed expenses" in connection with statutory accident benefits. The Financial Services Commission of Ontario (FSCO) is now accepting service provider licence applications through its website. Your business must apply for its service provider licence before August 31, 2014 to be provided a licensing decision by November 30,

3 Narrator: Today s webcast is the first of a regular series webcasts FSCO will be releasing. Each webcast within the series will focus on addressing frequently asked questions about service provider licensing. 4 The purpose of today s webcast is to answer technical support and troubleshooting questions relating to FSCO s online service provider licence application system. Over the next three minutes we will cover the following questions: What do I need before my business can apply for a licence? What Internet requirements should I have before applying for a licence online? How do I provide my Ontario mailing address for the business on the application? How do I update my contact information on my FSCO Account? How do I enable JavaScript? How do I enable cookies? How do I delete my history/cookies? How do I enable pop-ups? 4

5 Let s get started. First Question: What do I need before my business can apply for a licence? 5

6 Before Applying: Your business and all its branches, locations and/or facilities must be enrolled in HCAI. Your business must appoint a Principal Representative, as required by law, to complete the application and attestation. Ensure your business' Principal Representative has the following information on hand before submitting a service provider licence application: An Ontario mailing address that is suitable for receiving registered mail (not a post office box). An address for the business. Information about each facility, branch, or location registered with HCAI (such as: names, addresses and registration numbers) that will be listed under the service provider's licence. Articles of Incorporation or Partnership Agreements (for a corporation or a partnership). Details of trade names registered and used by the business and expiration date of their related master business licence registration. Your Canada Revenue Agency business number. Your Ontario business registration number, if applicable (typically found on the articles of incorporation or registration of the partnership. Contact information of all officers, directors or partners of the business including their address. The number of regulated and unregulated practitioners, by profession. The total number of unique statutory accident benefit claimants in respect of whom your business received payment in 2013 from an automobile insurer for an invoice submitted through HCAI. A valid VISA, MasterCard or American Express card, or a valid debit card. It s important to note that individual addresses that are provided on the application must be different and unique for each person identified. 5

7 What internet requirements should I have before applying for a licence online? It is highly recommended that applicants use Internet Explorer 9 and have the following Internet requirements in place before applying for a licence online: A secure high-speed internet connection via Cable, DSL or wireless access JavaScript enabled Cookies enabled Pop-ups enabled 124-bit encryption enabled 6

8 How do I provide my Ontario mailing address for the business on the application? In Step 3 of the application, you will see a magnifying glass on the right side of the field where the address has to be inserted. Click on the magnifying glass. 7

9 A new window will open. You must click on the magnifying glass on the right side of the address field. 8

10 Insert the postal code of your mailing address and click Next. 9

11 A list of addresses associated with the postal code you just entered will be displayed on the screen. You must first select your mailing address or the right series of street numbers from the list provided. You can then enter your street and/or suite number/suffix, as applicable, and click Finish. 10

12 The address field will be automatically populated with the information you provided. You can click Save to accept the address and proceed with the application. Please note that you cannot type in your mailing address in the field. 11

13 How do I update my contact information on my FSCO account? Updates to your FSCO account can only be made on or after December 1, Please log on to your FSCO account on after December 1, 2014 to update your contact information. 12

14 How do I enable JavaScript? In Internet Explorer, click on the Tools button of Menu bar and select Internet options. 13

15 From Internet Options window, select the Security tab. 14

16 Then, click on Custom level. 15

17 Locate Scripting near the bottom of the list. Under Scripting, Select Enable. Then, press OK. Click Yes for answer confirmation, and press Ok. Then, refresh/restart Internet Explorer. 16

18 How do I enable cookies? In order to enable cookies for FSCO, add FSCO to trusted sites. Click the Tools button, and then click Internet Options. 17

19 Click on the Security tab. Select Trusted sites and then click Sites. 18

20 Type in the given field, and press Add. Then press Close. 19

21 How do I delete my history and my cookies? In Internet Explorer, Go to Tools from the menu bar. Select Delete browsing history. 20

22 Select all the types you want to delete. Click on Delete, and all selected files will be deleted. 21

23 How do I enable pop-ups? In Internet Explorer, go to Tools from the menu bar. Select Pop-up Blocker. Under Pop-up Blocker, Select Pop-up Blocker settings. 22

24 Type in the given field, and press Add. Press Close. 23

25 Don t forget to apply for your service provider licence with FSCO before August 31, If you do not apply before that date you may not receive licensing decisions by November 30, Apply today. 24
