Our corporate office with a storage facility warehouse is located in Irving, Texas. CONFIDENTIALITY Photo-Stat carries a $2 million general liability with omission and errors policy. This policy covers the company against all losses contractually. Photo-Stat respects all confidentiality rights regarding patient information contained in medical records. A proper and legally valid authorization must be present before a request is processed. HIPAA COMPLIANT Photo-Stat has conformed to HIPAA security standards protecting the confidentiality and integrity and individual identifiable health information. Photo-Stat will sign medical facility s confidentiality statement and Business Associates PHI Privacy Agreement. QUALITY ASSURANCE PROGRAM Photo-Stat has a commitment to excellence in service that is monitored on a regular basis. We have established a process for measuring the quality of our service and make the measurements known to you, our customer. We ask all of our members to grade our performance of services so we can objectively review delivery of service, evaluate our performance and identify potential problems. EMPLOYEES All employees are trained by medical record professionals and obtain their Certified HIPAA Security Professional (CHSP) credentials. They are required to attend continuing education seminars during the course of the year. On going continuing education courses help provide knowledge and vision for the company and the facilities we serve. Qualified back up coverage is always available to guarantee continuous service in medical facilities. Field coordinators oversee members and employees and are held accountable for their performance. REFERENCES We service over 250 hospitals and doctor offices in the State of Texas. We have enclosed a copy of our current member list and also testimonials from some of our members in this brochure. Your bonded Photo-Stat Representative is HIPAA compliant and processes all of your facility s medical record requests on a daily or weekly basis at your office, including patient transfers, insurance, attorney, and disability and subpoena requests. Photo-Stat provides all equipment and supplies, pays postage for all medical record requests including certified and overnight mail, and offers notary services for subpoena requests. ELECTRONIC MEDICAL RECORDS Photo-Stat is familiar with all electronic medical records (EMR) programs and has solutions to enable medical facilities that have implemented an EMR system to utilize our services. All solutions are aimed at minimizing your office overhead while generating an additional revenue stream for your facility. Specifically, Photo-Stat will provide necessary staffing at your office equipped to retrieve requested medical records from your EMR for processing. Photo-Stat s secure and patented StatLink is installed for quick and easy use by our staff to process all medical record requests that can either be delivered via mail or electronically through a secure 256 bit encrypted link. Records can also be retrieved electronically through a secure VPN connection to avoid any disruptions to your office environment. Security reports are issued to medical facility administration identifying correct patient names and records that were extracted by Photo-Stat. Records are housed in a climate controlled HIPAA compliant warehouse protected by sprinkler systems and a 24/7 monitored alarm system giving you peace of mind that records are guaranteed for security. When an archived record is requested for medical record release purposes, we quickly pull the record for processing. Unnecessary delivery or access fees are eliminated amounting to a significant cost savings. STATUS CHECK This sophisticated web based tracking program enables authorized users to view the complete evolution of a medical record request. Features of this program include when a request was received, processed, checked for errors and delivered to whom, on what date and time. For records that cannot be delivered electronically, tracking numbers will be provided for verification of who signed for the records. Member facilities and specific requestors will be issued unique User Ids with protected passwords to ensure security.
