You can choose from the following options: Option 1 - $25,000 Option 2 - $50,000 Option 3 - $75,000 Option 4 - $100,000 Optional Life coverage is also available for your Spouse in the amount of $25, or $50,000 with Evidence of Insurability. Optional Life coverage is also available for your Child(ren) in the amount of $5,000 or $10,000. 8 About your Long-Term Disability Insurance By enrolling today, you are electing disability coverage that: pays you 60% of your monthly earnings, up to $5,000 a month, when your disability claim is approved covers you up to retirement age regardless of when you became disabled starts 90 days after your covered disability comes with convenient payroll deduction This policy contains limitations and exclusions that may affect benefits.
