2 INSTRUCTIONS FOR COMPLETING THE FIELD EVALUATION PROCESS The BSW Curriculum at Asbury University is organized around 9 competencies and 30 behaviors (consistent with CSWE 2015 EPAS). The Competencies are: Demonstrate Ethical & Professional Behavior; Engage Diversity & Difference in Practice; Advance Human Rights & Social, Economic, and Environmental Justice; Engage in Practice-Informed Research and Research- Informed Practice; Engage in Policy Practice; Engage with Individuals, Families, Groups, Organizations, & Communities; Assess Individuals, Families, Groups, Organizations, & Communities; Intervene with Individuals, Families, Groups, Organizations, & Communities; and Evaluate Practice with Individuals, Families, Groups, Organizations, & Communities. The Student and Agency Supervisor are encouraged to use the Field Learning Contract, as a basis to identify Field Learning Tasks for all 30 of the Behaviors. This Field Evaluation Form is completed at the end of the semester. During the semester, the student and the agency supervisor can, and is encouraged to, informally discuss the student s progress on the field learning tasks. This can be a basis for identifying a student who is experiencing difficulty in the field placement, so a mid-semester meeting can be scheduled with the faculty field professor. This type of meeting is important for the student, agency supervisor and faculty field professor to become aware of specific difficulties in order to allow time for a re-focusing of efforts, if needed. Prior to the start of the field placement experience, the students will have reviewed ALL the competencies and behaviors, in the field seminar classes. As the agency supervisor, you can view your student as the expert on these competencies and behaviors and you will contribute your expertise to the evaluation process by knowing your agency and actively engaging your student in the evaluation process. The faculty field professor will be available to assist you in ALL the steps of the process. LEARNING CONTRACT (INCLUDING TASKS WORKSHEET) The evaluation process for the student should begin when the student starts the field placement, by completing the Field Learning Contract. The following are steps to follow to complete the Learning Contract: (1) As the agency supervisor, you and your student will complete the beginning pages of the Learning Contract. (2) Next the agency supervisor will review the tasks that are already checked on the worksheet these are tasks that should be relevant to MOST agencies. If it is NOT relevant then it can be crossed off on this worksheet. (3) Next the agency supervisor will review the remaining tasks on the worksheet and check ones that are relevant to the agency/field site. For example the tasks that start with the word discuss should be possible in most setting by having the student bring relevant content to discuss from their classes. (4) Lastly both the agency supervisor and student will sign the agreement on the last page, and the student will be responsible to provide a copy for the agency supervisor. (5) The original is given to the seminar professor and the faculty field professor maintains the ORIGINAL copy for his/her use. 2

3 FIELD EVALUATION FORM The evaluation process should reflect an ever-increasing ability to do self-directed practice, and students should be able to assume greater autonomy and a more challenging caseload, with clinical supervision, as the semester progresses. This growth should be understood through beginner generalist bachelor level social work. Transferring Tasks from Worksheet to Evaluation Form Once the Learning Contract is completed (including the tasks worksheet), it will be the responsibility of the student to transfer this information from the Worksheet to the Field Evaluation Form. The student will check appropriate tasks in the 2 nd column of the Field Evaluation Form. The student will have time in a field seminar class to complete this process. Field Evaluation When this process is completed, you as the agency supervisor can begin to evaluate the student s progress, based on these tasks. The following are steps to follow to complete the field evaluation form for the semester: (1) During supervision take time to review the evaluation form with your student and identify AT LEAST ONE TASK that will be the basis for evaluating EACH BEHAVIOR i.e. ALL 30 behaviors. If a relevant task is NOT identified, there is space in column on the field evaluation form, in order to briefly (a few words) identify a relevant task in your agency/. When you have completed this process for a student, you can use this same task as you work with Asbury University students as well as students from other BSW/MSW programs. (2) PRIOR TO THE VISIT BY THE FACULTY FIELD PROFESSOR, the agency supervisor with the student will discuss each of the 30 behaviors and relevant tasks and decide on a rating from 1-5. The rating scale is at the top of each page of the evaluation form. (3) DURING THE MEETING WITH THE FACULTY FIELD PROFESSOR, the field evaluation form will be reviewed. All behaviors MUST BE EVALUATED. The faculty field professor will also assist the agency supervisor to complete the summary form for the semester, which will be the basis for determining 50% of the grade for Field Practicum. The evaluation form WILL BE SIGNED at this meeting. Copies will be made as follows: a. Faculty Field Professor ORIGINAL b. Agency Supervisor COPY c. Student COPY 3

4 Summary Page The last two pages of the field evaluation form include the overall student rating. Please indicate on the chart the Global Competency Ratings from (1 5), the student received for each of the 10 Competencies (Competency Practice is divided into 4 subparts a, b, c, and d. Add up the total and divide by the number of Competencies ratings indicated on the chart, in order to get the average (mean rating). This average (mean rating) will be the Overall Rating for the semester, and the Faculty Field Professor will circle the appropriate grade on the Grading Chart. YOUR FACULTY FIELD PROFESSOR CAN ASSIST YOU IN COMPLETING THE CHART ON THE SUMMARY SHEET. NOTE: The grade for Field Instruction has two components, the actual score given by the Agency Supervisor and Faculty Field Professor; and the score given by the Field Seminar Professor, who is responsible for recording the final grade. Your careful attention to the completion of this evaluation form and your feedback on our students progress in Field Education are greatly appreciated. 4
