Tools for public health NICE Public Health Guidance Steve Sparks Associate Director NICE June 26 th 2012

What is NICE? The National Institute for Health and Clinical Excellence (NICE) is the independent organisation responsible for providing national guidance on the promotion of good health and the prevention and treatment of ill health.

The role of NICE To identify good clinical and public health practice using the best available evidence To help resolve uncertainty for the public, patients and professionals To reduce variation in the availability and quality of practice and care

Who writes the guidance?

Core principles of all NICE guidance Comprehensive evidence base Expert input Community, patient and carer involvement Independent advisory committees Genuine consultation Regular review Open and transparent process

NICE public health guidance Smoking in the workplace Reducing STI and under 18 pregnancies Reducing substance amongst vulnerable young people School-based interventions on alcohol Knowledge, attitude and behaviour change Smoking cessation services Community engagement and Stopping children starting smoking community development approaches to health improvement Managing long term workplace sickness Maternal and child nutrition Social and emotional wellbeing in primary education Workplace physical activity

Audiences for public health guidance Local government Workplace / employers The NHS Education The utilities Private and voluntary sectors Other arms of government The public

Why use NICE guidance? Based on the best available evidence Effectiveness: what works and in what population Cost-effectiveness: value for money approaches to national and local priorities Reduce variation and inequalities Improve local services and accountability Improve health and wellbeing outcomes Supports the case for investment Supports local integration discussions and decisions with partners on investment and prioritisation Fit with broader policy agenda.

Public Health Outcomes Framework

2.3 Rate of smoking at time of delivery per 100 maternities NICE Brief interventions and referral for smoking cessation, Quitting smoking in pregnancy and following childbirth 2.7 Crude rate of hospital emergency admissions caused by unintentional and deliberate injuries in age 0-17 years, per 10,000 resident population NICE Preventing unintentional injuries among under- 15s in the home, and on the roads

Main barriers to implementation Practioner distrust Lack of organisational support - structures and processes Resources (or lack of them) Poor knowledge of support from NICE

Practical support from NICE Quality Standards Practical support tools (audit / costing / education... ) Public Health Briefings NICE Pathways Evidence in health and social care Apps

NICE Quality Standards Progress on outcomes will be supported by quality standards. These will be developed for the NHS Commissioning Board by NICE, who will develop authoritative standards setting out each part of the patient pathway, and indicators for each step. NICE will rapidly expand its existing work programme to create a comprehensive library of standards for all the main pathways of care. The standards will cover areas that span health and social care.

180 healthcare quality standard topics

NICE help for OSCs Why should OSCs review the impact of local authority and health and wellbeing strategies on unintentional injuries in children and young people under 15? What are the likely costs and savings, and does implementation offer value for money? What information do OSCs need to prepare for the review? Ten questions to ask

Key facts for local councillors What will it cost and what will we save? Simple and cheap measures that have been proven can save lives and prevent children from serious disfigurement or disability. For example, 20 mph zones have been shown by several studies to reduce casualties by around 40%. A cost benefit analysis of local road safety engineering schemes, including 20 mph zones, often give first year rates of return of about 100 per cent; that is the costs are recovered by the value of death and injury saved in the first 12 months.
