3 The following is a summary of the in-built and optional extra benefits available in Zurich Income Replacement. Please read the Zurich Wealth Protection PDS for all terms and conditions, including relevant exclusions. In-built benefits Income benefit there are two types of Income benefit agreed value and indemnity. If indemnity is chosen a premium discount will apply. Under super ownership, the income benefit must be indemnity. Qualifying for an Income benefit (agreed value or indemnity) the Income benefit is payable after the expiry of the waiting period if, solely as a result of a sickness or injury until the expiry of the waiting period: the life insured s pre-disability income from his/her usual occupation reduces by 20 per cent or more, or the life insured is unable to perform one or more income producing duties of his/her usual occupation, or the life insured is unable to perform the income producing duties of his/her usual occupation for more than 10 hours per week. To be eligible for an Income benefit, the life insured must be under the regular care of, and following the advice of, a medical practitioner. If the standard level of cover is selected, the life insured must also stop working for at least 14 days during the waiting period. If at the expiry of the waiting period the life insured has no post-disability income or is working for 10 hours or less per week then: if the policy is agreed value, we will pay the insured monthly benefit if the policy is indemnity, we will pay the lesser of the insured monthly benefit and 75 per cent of pre-disability income. Otherwise, the Income benefit we will pay after the expiry of the waiting period will be proportionate to the loss and calculated on a monthly basis using the following formula: pre-disability income post-disability income pre-disability income x insured monthly benefit If the cover is indemnity, the benefit will be capped so that the amount we pay does not exceed 75 per cent of the life insured s pre-disability income. We will continue to pay the Income benefit until any one of the following events occurs: the sickness or injury giving rise to the claim does not prevent the life insured from earning their pre-disability income from personal exertion in their usual occupation the benefit period ends the policy ends the death of the life insured the life insured is no longer under the regular care of a medical practitioner with regard to treatment for the sickness or injury the life insured is not following the treatment recommended by a medical practitioner at such intervals and frequency as will lead to a cure, alleviation or minimisation of the condition causing their disability. In the event of a claim, Zurich will offset: other disability income policies not disclosed to us at the time of underwriting and workers compensation or other legislated benefits (does not apply to A1, A1M or A2 occupations). The life insured can earn up to 10 per cent of pre-disability income in the first three months without any offset. Under super ownership, the life insured must cease work for at least one day, sick leave is offset and total benefit plus post-disability income is capped. You should refer to the PDS for more information. Specified injury benefit the waiting period is waived and the benefit is paid immediately for a fixed period if the life insured suffers from a specified injury (whether or not a loss of income results). The benefit covers specific conditions and injuries. Rehabilitation benefit Workplace modification Zurich will pay up to three times the monthly Income benefit for approved expenses incurred in carrying out workplace modifications Rehabilitation program Zurich will provide an additional 50 per cent of the monthly Income benefit each month towards the expenses of an approved rehabilitation program for a maximum of 12 months Rehabilitation costs Zurich will reimburse up to twelve times the monthly Income benefit to cover approved rehabilitation expenses. Funeral benefit three times the insured monthly benefit will be paid if the life insured dies while the policy is in force. Page 3 of 6
4 Flexible cover benefit if the life insured s income reduces while pregnant or following the birth or adoption of a child, the insured monthly benefit can be reduced and then reinstated without reassessment of health within two years. The maximum reduction is 75 per cent and the reinstatement of agreed value cover depends on a return to full-time paid employment. Waiver of premium benefit Zurich will waive or refund all premiums when any Income benefits or Specified injury benefits are payable. If the claim form is lodged within 30 days, premiums paid in the waiting period are also refunded. Confined to bed benefit if the life insured is confined to bed because of sickness or injury for more than two days in a row and during that period requires the full time care of a nurse or personal attendant, Zurich will pay one thirtieth of the monthly benefit for each day the life insured is disabled during the waiting period, to a maximum of 180 days. Special care benefit this includes four benefits which may be payable after the end of the waiting period, if an Income benefit is being paid, to cover the following situations: if a direct family member has to give up work to look after the life insured if the life insured is confined to bed or to his/her house and dependent on home assistance or nursing care if a direct family member has to stay 100km or more away from home to be with the life insured if the life insured s sickness or injury occurs overseas and he/she chooses to return to Australia. Family support benefit if the life insured is receiving an income benefit and dies before the policy expiry, Zurich will continue to pay the spouse an Income benefit for up to 12 months. Trauma benefit if the life insured suffers a specified trauma, Zurich will pay a lump sum equal to six times the monthly Income benefit. Future insurability (in-built benefit under premier only) the insured monthly benefit can be increased by up to 15 per cent every policy anniversary up to age 55 without further underwriting (provided no claims have been made) subject to cover not exceeding 75 per cent of income. In-built provisions Interim cover while Zurich is assessing the application, we will provide interim cover for up to 90 days against accidental injury which results in the life insured being unable to work and earn any income. Inflation protection Zurich will offer to index the insured monthly benefit each year in line with inflation without further assessment of health. Recurrent disability if disability recurs within 12 months of the life insured returning to work, a further waiting period will not apply. Zurich will start paying an Income benefit again immediately. Concurrent disability if more than one separate and distinct sickness or injury results in a disability, payments will be based on the policy condition that provides the highest benefit. Premium holiday the policy owner may request a 3, 6, 9 or 12 month break in cover (maximum 12 months over the life of the policy). No cover is provided for any insured event apparent or occurring during a premium holiday. Optional extras Increasing claims option the income benefit will be increased quarterly to keep up with inflation while on claim. Superannuation contributions option Zurich will cover up to 100 per cent of the life insured s superannuation contributions should they qualify for an Income benefit, Specified injury benefit, Confined to bed benefit, Trauma benefit or Day 4 accident benefit. Business expenses option if the life insured is disabled and suffers a loss of income, Zurich will pay up to 100 per cent of the fixed costs of running the business. Lump sum accident option a lump sum is payable if the life insured is involved in an accident which causes their death or a listed injury within 180 days of the accident and before the policy anniversary following the life insured s 65th birthday or the benefit expiry date (whichever is earliest). Day 4 accident option (with waiting periods of 90 days or less) if the life insured is disabled due to injury for more than three consecutive days then benefits will be paid during the waiting period on a daily basis (one thirtieth of the monthly benefit). The benefit payable is backdated to day one. Trauma option if the life insured suffers a specified trauma and is disabled as a result Zurich will double the income benefit payable for up to six months. If the life insured dies the Funeral benefit will also be doubled. Page 4 of 6
5 Future insurability option the insured monthly benefit can be increased by up to 15 per cent every policy anniversary up to age 55 without further underwriting (provided no claims have been made) subject to cover not exceeding 75 per cent of income. Booster option an additional one third of the Income benefit will be paid for the first 30 days of claim if we are paying the maximum benefit available under the policy. Family care option if an Income benefit is being paid and the life insured dies before the policy expiry, Zurich will continue to pay the spouse an Income benefit for up to five years after the life insured s death. Severe disability option if the life insured has been unable to do two of the activities of daily living for at least three months Zurich will increase the monthly benefit payable by one third while the condition continues. Needlestick cover option for an additional premium your eligible clients can receive a lump sum of up to $1,000,000 (multiples of $50,000 only) if they occupationally acquire HIV, Hep B or Hep C. Home support option recognises the contribution of a spouse who is not in paid work or is working part time. It pays a monthly living expenses benefit, benefits to reimburse domestic assistance and child care costs and a package of rehabilitation benefits. Other product features Tailored super structuring it is possible to have two related Zurich Income Replacement policies (where one of the policies is owned by the trustee of a superannuation fund) or a Zurich Income Replacement policy related to a Zurich Superannuation Income Replacement policy. This allows a mixture of super and ordinary benefits to be linked. Linked benefits provide a cost-effective package of cover overall, but can normally only be selected within a single policy. Related policies remain as two separate policies but will behave as if all linked benefits are combined on one policy, which means that a claim on one policy may impact the other. Within the related policies structure, cover can be set up as an: Indemnity income benefit (in super) with built in benefits and extra cost options payable through super and Income superlink benefit (outside super) with built in benefits and extra cost options not payable through super The cover provided under each policy is allocated to optimise alignment with superannuation law payment rules. Both policies must have the same level of cover (standard, comprehensive or premier) and benefits may be payable from either the related super policy or a combination of both related policies. Unemployment and employment breaks (non SR occupations) if the life insured is involuntarily unemployed Zurich will waive the premiums for up to three months at a time (12 months over the life of the policy) provided the life insured registers with an approved employment agency. The policy must first have been in place (without any periods of waived premium) for at least 12 months. Refer to the PDS for terms of cover during periods of unemployment or during employment breaks. Worldwide cover if the life insured is outside Australia during a claim the life insured must undergo an annual medical in Australia, or in another country, by a doctor nominated or approved by Zurich, every 12 months for the claim to continue. Exclusions Zurich will not pay an Income benefit if loss of income is the result of: intentional self-inflicted act attempted suicide uncomplicated pregnancy or childbirth elective surgery or donor transplant surgery within six months of the cover start date an act of war (whether declared or not) any event or medical condition specified as an exclusion in the Policy schedule.
