1 Funding Opportunity Application Worksheet Thank you for your interest in the Presidential Youth Fitness Program (PYFP) funding opportunity. 1 This worksheet is provided to help you gather the information you need in order to complete your application online by the deadline. Please note that completing this worksheet does not qualify as an application and will not be accepted, it is simply provided as a tool to help you. Some important points to remember: This application worksheet may not be valid for the current funding opportunity announcement. Check the program website ( to make sure you have the most up to date version as indicated by the date at the bottom of this page A school must adhere to certain terms and conditions in order for its application to be considered. For a list of the terms and conditions, please visit the program website The online application and required memo must be submitted within the application period (January 1- February 9, 2015). Fuel Up to Play 60 schools interested in applying in the Fall 2014 funding round, please visit for application information. The District Application is slightly different. If you are interested in applying on behalf of your district, please send a message to info@presidentialyouthfitnessprogram.org. Please review the Frequently Asked Questions document to understand the purpose and extent of this three year funding opportunity. There are costs a school will be required to cover as part of its participation. Eligibility Criteria Public and private schools employing a certified physical education teacher are eligible to apply. PreK/K schools or those serving only students in grades K- 2 or K- 3 are NOT eligible. Schools required to submit FITNESSGRAM data for district or state reporting purposes using their existing FITNESSGRAM software are NOT eligible to apply. Competitive Preference All Schools that sign up to become or are a Let s Move! Active School (LMAS) will receive one bonus point in the review process. 3 3 enhancement and if so, what is the name of the training Does your school district or district wellness policy specify minutes of weekly physical education and is this achieved Does your school have an active school wellness committee or council Does your school qualify for Title 1 status and, if so, what percentage of students qualify for free/reduced lunch Current Fitness Assessment Information If fitness assessments are conducted, which one is used and how often during the school year Is conducting fitness assessments written in school policy Are any grades assessed every year and if so, which ones How many students are assessed each year Are student results shared with students, parents or guardians Are students recognized for their performance on the fitness assessment Additional Resource Needs Applicant will need to: Provide the quantity (up to 2) and level of the Physical Best Activity Guide (elementary or middle/high or 1 of each) Indicate wish to receive a copy of The Brockport Physical Fitness Test Manual (ebook) Budget Narrative Be prepared to address in 500 words or less how the following costs will be covered: Substitutes that may be needed for teachers to participate in the virtual trainings (a self- paced course spread over 2 years and requiring approximately 7 hours per teacher to complete or 5 hours spread over 2 years if teacher(s) participated in the PYFP in- person training) Instructional materials (e.g., CD player, gym mats, scale, cones, etc.), if the school does not have access to them already 100% of the costs in Year 4 and beyond Program Narrative Based upon current understanding, but realizing the approach may change once the required professional development training has been completed, be prepared to identify the time of year the assessment will be conducted and in what grades while keeping in mind it is recommended that every student be afforded the opportunity to benefit from an annual fitness assessment process. In 250 words or less, be prepared to share information on how the Presidential Youth Fitness Program will be implemented. Responses should demonstrate knowledge on assessing students with disabilities and plans for recognizing students or interest in learning about the elements and methods for doing so. 4 pyfp.org Presidential Youth Fitness Program Funding Opportunity 1 Frequently Asked Questions What is the Presidential Youth Fitness Program The Presidential Youth Fitness Program (PYFP) is a voluntary program that promotes a quality fitness education and assessment process within the context of a school s existing physical education program. What is the Purpose of this Funding Opportunity This funding opportunity is designed to facilitate participation in the PYFP by providing schools with the supplemental resources that support the PYFP model. These resources include: 1. Assessment: FITNESSGRAM site license and companion materials including The Brockport Physical Fitness Test Manual (ebook) and access to test videos 2. Professional development: Virtual courses for self- paced learning and access to a professional learning community 3. Awards and recognition: Presidential Youth Fitness Awards for students in grades K- 4 and PYFP Fitness Club items for students in grades K- 3 Thanks to a generous donation from the General Mills Foundation, this funding opportunity will support participation in the Presidential Youth Fitness Program for a period of three academic years starting in fall 2015 and ending in spring Schools that are selected to receive funding will not receive a check to purchase materials or supplies. The elements identified in 1-3 above will be sent directly to the school from the supplier. See Is there a Cost for more information on this support. Who is Eligible Public and private schools employing a certified physical education teacher on a full or part- time basis are eligible to apply. Please check with your district coordinator before applying if you are already using the FITNESSGRAM software in your school or as part of a district- wide FITNESSGRAM software installation. The funding requires that a school use the PYFP web- hosted version of the software. Schools serving only students in grades K- 2 or K- 3 are not eligible. Can School Districts Apply Yes! School districts are encouraged to apply. 5 Is there a Cost This funding opportunity will cover 100% of the cost for the items listed in 1-3 above in years 1 and 2. In year 3, 100% of the costs for the software and professional development will be provided. Schools will be given a $100 credit for recognition items of their choosing. See Value of this Award for more information. There may be a cost associated with teacher participation in the required virtual trainings (a self- paced course spread over 2 years that takes approximately 5-7 hours to complete) and for materials of instruction (see What this Funding Opportunity Will Not Cover for details). When Can Schools Apply On- line applications for Round 3 will be accepted for the following: Fuel Up to Play 60 Schools: August 26- November 1, 2015 All Schools: January 1- February 9, 2015 (11:59 PM PT) How Do Schools Apply Applicants are encouraged to complete the application worksheet, first. This will reduce time needed to complete the on- line application (accessible at or allow someone to complete the on- line application on their behalf. All questions on the application must be answered. One application must be completed for each school. The school s principal and IT manager must agree to the terms outlined in the web hosting memorandum of understanding (copy available on pyfp.org). A memo of support (template provided on website) signed by the principal, physical education teacher, and IT manager must be uploaded and submitted with the application. Please review your school s policy for sharing de- identified, aggregate data for end of year program reports. Schools must ensure they are in compliance with district policies for data submission. Required data includes, but may not be limited to, information supplied by a FITNESSGRAM 10.x software- generated report. Failure to submit end of year reports may result in cancellation of support. What are the Requirements for Schools Selected to Receive Program Support 1. IT Manager and Physical Educator(s): Watch the FITNESSGRAM software training webinar 2. Physical educators complete virtual course and sustainability webinar (approximately 8 hours total) 3. Make plans for and conduct the FITNESSGRAM assessment according to the training 4. Recognize student achievement in fitness and physical activity in accordance with the program s guidance 5. Affirm participation in Year 2 and 3

Participate in evaluation activities, including submission of required data and completion of a brief questionnaire on an annual basis (see information under How do schools apply to ensure compliance with district policies on sharing de- identified aggregate data). 7. Repeat numbers 2-6 in Year 2 and 3 Secure funds or write into annual budget costs for continuing the Presidential Youth Fitness Program starting with Year 4. What this Funding Opportunity Will Not Cover Purchasing instructional materials used to conduct the various tests in the FITNESSGRAM assessment (see Additional Information) Substitute costs associated with physical education teacher participation in virtual trainings. A project nor grant manager. Additional Information: There are instructional materials that are routinely used to conduct the FITNESSGRAM assessment (e.g., gym mats, cones, scales, audio equipment to play a CD or a stop watch and whistle, calipers, masking or athletic tape, and sit and reach box. Instructional materials that may be used are dependent upon the test item offered and preference of the educator. Value of this Award The following cost breakdown is for a school employing two physical educators and having 500 students. The total support offered through this three year funding opportunity is approximately $2553. The one- time cost to set up the subscription is covered by this funding opportunity in Year 1, so a school would not have to incur that cost should they continue with the software subscription after the three year funding period ends. Professional development is a variable cost that depends on whether new teachers are hired who have not completed the Success with the Presidential Youth Fitness Program virtual training package or completed the PYFP in- person training. A school could choose to pay roughly $70 to purchase a virtual course for a teacher to stay up to date or enhance knowledge in a particular area addressed through the Presidential Youth Fitness Program. Awards will remain approximately the same from year to year unless a school chooses to purchase one of the other award options (a digital badge, medal, t- shirt, etc.). *Please note, all prices are subject to change.
