3 AED Description Small, Portable, Automated Device used on individuals in Sudden Cardiac Arrest Recognizes Ventricular Fibrillation (VF=SCA)-deadly cardiac rhythm Automatically Delivers Shock when Appropriate or Advises Individual to Push Shock Button Very Easy to Use May be used by Laypersons

4 What does a Cardiac Arrest Look Like????

5 Sudden Cardiac Arrest Life Threatening Event Collapse with No Warning V-Fib-Heart is Quivering & Cannot Pump Oxygenated Blood No chance of Survival without early CPR and AED 7-10% Decrease in Survival with each Passing Moment

6 Sudden Cardiac Arrest Occurs more often in Adults over 50 Often no Prior Signs or Symptoms Prior SCA= likelihood of repeat occurrence Causes may include Congenital Defects/Anomalies (previously diagnosed or undiagnosed) Electrical Cardiac Abnormalities Commotio Cordis-sudden impact to chest by object often during athletic events in children or adolescents

7 The Need for PAD There are an Estimated 250, ,000 SCAs in the U.S. each Year In Maryland there are approximately 4,000 SCAs each year. The national published median for survival from SCA is 6.4% In the Maryland PAD program ROSC* is 22% for all SCA and 29% for witnessed. *Return of Spontaneous Circulation Report to the MD General Assembly Re:Placement of AEDs. 8 The Evidence for PAD Literature demonstrates that: Training and equipping volunteers to attempt early defibrillation within a structured response system can increase the number of survivors to hospital discharge after out-of-hospital cardiac arrest in public locations. Trained nonmedical responders can use AEDs safely and effectively. N Engl J Med Aug 12, 351(7): J Cardiovasc Nurs Nov-Dec; 19(6):384-9

9 Sudden Cardiac Arrest Factors that increase the likelihood of survival from Cardiac Arrest include: Witnessed (Casinos > 70% survival) Cause (medical/cardiac origin vs. trauma) Time! Time! Time!

10 Maryland AED Law Originally Passed in 1999 Administered by MIEMSS under the authority of the EMS Board Annotated Code of MD Education Article COMAR updated in 2013

11 Summary of 2013 Changes Equipment Checks now monthly instead of weekly. AEDs must be visible and accessible to anyone willing to use them. Signage advising only trained personnel can use the AED must be removed. Training by a national organization in accordance with AHA Emergency Cardiovascular Care Guidelines.

12 Public Access Defibrillation ( PAD ) in MD Must Meet Requirements such as: Register and obtain certificate from MIEMSS (online registration coming soon!) CPR/AED Training for expected operators Monthly Equipment Checks (Batteries & Pads) AEDs must be visible and available to anyone willing to use Submission of Cardiac Arrest Report Form after an event

13 MD PAD Training CPR/AED Training Completed through a nationally recognized organization that teaches according to the current AHA ECC Guidelines Refresher course as required by training organization Certification not required

14 MD PAD QA Measures Maintenance of AEDs Monthly Inspection Records Training Dates Cardiac Arrest Reports FDA Report for Malfunctions

15 MD PAD Immunity from Civil Liability from AED law and Good Samaritan Act Facilities that have satisfied requirements and have a valid certificate Individuals acting in good faith
