4 Program Description Minnesota Senior Health Options (MSHO): MSHO is a voluntary managed care program that covers health care and Elderly Waiver services for people who: are age 65 and over, have Medical Assistance, and have both Medicare Parts A and B. Our MSHO program is called UCare s MSHO. There are no added costs for you to join this program. While a member of the plan, your Medical Assistance and Medicare Part A, B, and D will be obtained through the plan. For more information or to enroll in UCare s MSHO call UCare s Sales Department at the numbers listed on page i. If you choose to leave MSHO, you will be enrolled in an MSC+ plan for your Medical Assistance services, if you are eligible to enroll. Important Information Clinics The clinic information printed in this listing was correct as of July You need to choose a primary care clinic when you enroll. Your primary care clinic can provide most of the health care services you need, and will help coordinate your care. Please confirm with UCare s Sales Department that your clinic is still a provider with our health plan before signing up. If you DO NOT currently live in a nursing home, you must choose a primary care clinic from the listing in this booklet. Then write the six-digit primary care clinic code on your enrollment form. The code is found under the clinic s name. If you DO currently live in a nursing home that is in the UCare network, you may choose to receive your primary care in one of two ways: 1. In the nursing home setting, by choosing one of the Visiting Clinics listed in this booklet under the heading Nursing Homes by Visiting Clinics, or 2. In the clinic setting, by choosing a clinic from the Primary Care Clinic listings in this booklet. If you are a UCare s MSHO member and cannot find your primary care clinic or nursing home in this listing, or if you have questions, call UCare s Sales Department at the numbers listed on page i. Health Care Homes Health Care Homes (also known as Medical Homes) are health care providers that have been certified by the State of Minnesota. The program is designed to provide additional care coordination for members who need additional help with chronic or complex health care conditions. There is a process that members must follow to participate with a Health Care Home in our network. Contact any of the Health Care Home providers included in this listing for more information about their program. Cost Sharing You may be required to pay an amount toward some health care services. This is called cost sharing. You are responsible to pay your cost sharing amount to your provider. Members will receive a Member Handbook with more information about cost sharing. ii

5 Member Identification (ID) Cards and Member Packet Members will get a health plan Member Identification (ID) Card. This card must be shown whenever you get health care services including getting a prescription filled. You will receive your ID Card and a new member packet containing more information about UCare and your benefits soon after you are enrolled. Service Accessibility If you need special access to get services from a provider, you can get information by calling UCare Customer Services at the number listed on page i. This could include, but is not limited to, access for services such as availability of flexible hours, wheelchair access, or parking lot access. Service Authorization Our approval is needed for some services to be covered. The approval must be obtained before you get certain services or before we pay for them. For those services, your provider will be required to obtain prior authorization from us. To see a doctor or specialist who is not in UCare s network, you or your Primary Care Provider must obtain a service authorization from UCare in order for those services to be covered. Members will receive a Member Handbook with more information about which services require authorization. Services Care Coordination When you enroll in the MSHO program, you choose a primary care clinic. If you do not select a primary care clinic, you will be assigned one. After you enroll, you will be assigned a UCare Care Coordinator, who will offer you an initial health risk assessment and will work with you to coordinate the care and services you need. Your UCare Care Coordinator s job is to help you understand your benefits and get the most out of your benefits with the least amount of hassle and paperwork. Whether you re at home, in the hospital, or in a nursing home, you ll have one person to contact who understands and makes it easier for you to receive the services you need. You, your doctors, and others providing your care will all work together with your UCare Care Coordinator. Whenever you have questions, concerns, or need services, you can simply call your UCare Care Coordinator. If you are getting home and community-based services through the Elderly Waiver at the time you enroll, you will get these services through your UCare Care Coordinator. If you reside in the community and are not getting home and community-based services through the Elderly Waiver at the time of enrollment, your first screening will include an assessment of your need for these services. If you are assessed as eligible for home and community-based services under the Elderly Waiver, your Care Coordinator will work with you to get these services. Members in the MSHO program residing in the community or in nursing homes will be assessed annually for their ongoing care needs. You can get more information on care coordination services by calling UCare Sales at the numbers listed on page i. Chemical Dependency Chemical Dependency services provide care and treatment for chemical substance abuse, such as abuse of alcohol, narcotics or other drugs. UCare uses a network of chemical dependency providers in your community. You can get current information about those providers by using the Find a Doctor feature on ucare.org or by calling UCare Customer Services at the numbers listed on page i. After hours and on weekends, members can call the UCare 24/7 nurse line for assistance with finding provider information. 6 Chiropractic Care Chiropractic services are offered through Chiropractic Care of Minnesota, Inc. A list of providers will be in the member packet you ll receive when you enroll with UCare. You can get more information on how to access chiropractic services by calling UCare Customer Services at the numbers listed on page i or by using the Find a Chiropractor feature on ucare.org (found under the Find a Doctor tab). Dental We contract with Delta Dental of Minnesota (Delta Dental) to manage our dental benefits. If you have questions about any of your dental benefits, call the UCare Dental Connection at toll free or TTY at toll free, Monday Friday, 7 a.m. to 7 p.m. UCare s Dental Connection helps our members manage their dental care with one simple phone call. The UCare Dental Connection can help you: --Find a provider or dental home. --Schedule dental appointments, including appointments for follow-up and specialty care. --Coordinate transportation to dental appointments. --Coordinate interpreter services for dental appointments. --Answer questions related to your dental care. Durable Medical Equipment (DME) Durable medical equipment is any medical equipment used in the home to aid in a better quality of living and provide therapeutic benefits to a patient with certain medical conditions or illnesses. Examples are nebulizers, wheelchairs, crutches and many others. You can get information on Durable Medical Equipment by calling UCare s Sales Department at the numbers listed on page i. Elderly Waiver Home and Community Based Services If you are age 65 or over and need help to stay in your home, there is a program called Elderly Waiver (EW). EW offers more services than regular Medical Assistance. Services may include visits by a nurse, home care services, homemaker services, chore services, and more. Your Care Coordinator can arrange these services for you. If you would like more information, please call UCare s Sales Department at the numbers listed on page i. After you enroll, call the Customer Services number on the back of your UCare member ID card. Elderly Waiver Obligation If you get EW services and your income is over the income limit, you may have to pay a monthly amount. This is called the EW waiver obligation. Your county will notify you of any EW waiver obligation when your eligibility is determined. Health Education Quit smoking or chewing tobacco with the help of counselors at our tobacco quit line. The tobacco quit line provides counseling done over the phone. There is no charge. Call the tobacco quit line at toll free. If you are hearing impaired, please call the TTY line at toll free. Home Care Home Care services include skilled nursing, rehabilitation therapies (for example speech, physical, respiratory, and occupational), home health aide, medical equipment and supplies, Personal Care Assistant (PCA) and Home Care Nursing (HCN) services. For more information, including how you can access home care services, please call UCare Customer Services at the numbers listed on page i. iv

7 s The hospitals in the listing are contracted with UCare. If you don t know which hospital your doctor uses, ask him or her. Unless it is an emergency, you should use a hospital that is in UCare s provider network, and is associated with your doctor. Remember to call your primary care clinic before going to the hospital, except in an emergency situation. You can get more information on hospital services by calling UCare Customer Services at the numbers listed on page i. Interpreter Services Spoken language and sign language interpreter services are available to help you get services. Oral interpretation can be given to you in your language. You may be able to get written information in your language. Face-to-face oral language interpreter services are only covered if the interpreter is listed in the Minnesota Department of Health s Spoken Language Health Care Interpreter Roster. UCare members can call UCare Customer Services at the numbers listed on page i. After-hours and weekend calls are answered by the UCare 24/7 nurse line. Mental Health Mental health services provide care and treatment for problems that affect your thoughts, mood and behavior, like depression, anxiety or other mental health issues that interfere with your daily functioning. UCare uses a network of mental health providers in your community. You can get current information about those providers by using the Find a Doctor feature on ucare.org or calling Customer Services at the numbers listed on page i. After hours and on weekends, members can call the 24/7 nurse line for assistance with finding provider information. The phone number for the nurse line can be found on the back of your Member Identification card. Nursing Home and Skilled Nursing Home Care We will cover a total of 180 days of nursing home care. If you need nursing home care beyond the 180 days, the Minnesota Department of Human Services (DHS) will pay for your care. This applies unless you were already in the nursing home when you enroll in our plan. You can get more information on nursing home services through your Care Coordinator when you become a member. Please refer to the nursing homes section in this listing to find nursing homes that participate in UCare s plan. Open Access Services You may choose to receive care from any physician, clinic, hospital, pharmacy, or family planning agency to get some services even if it is not in our network. The services are: family planning, diagnosis of infertility, testing and treatment of sexually transmitted diseases, and testing for AIDS or HIV-related conditions. You can get more information on how to access open access services by calling UCare Customer Services at the numbers listed on page i. Pharmacy Members must use our network of pharmacies for drugs covered by our health plan. You will get a list of pharmacies in the member packet you receive when you enroll with UCare. The drug must be on our covered drug list (formulary). You can visit ucare.org for the most current formulary and pharmacy network information. Remember to have the following available when you call to refill your prescription: UCare s MSHO Member Identification (ID) Card Minnesota Health Care Programs card Cards for any other health coverage you have, such as Medicare Part D or private insurance It is important to remember to show both your Member Identification (ID) card and your Minnesota Health Care Programs card when picking up your prescriptions. 8 UCare Customer Services at the numbers listed on page i. Specialty Care Specialty Care describes services you can get from doctors or surgeons whose training focuses primarily in one specific field of medicine, such as neurology, cardiology, or oncology, among others. You can get more information on how to access specialty care by calling UCare s Sales Department, from the Provider Directory you will receive when you enroll with UCare, or by using the Find a Doctor feature on our website at Transition of Care When you join UCare, we will help you continue to get necessary health care services that were authorized before you enrolled with us. Some examples of these services are: orthodontia, mental health, chemical dependency, at-risk pregnancy, or prescriptions. If you are currently receiving care from a provider who is not in our provider network, we will help you transition to a participating provider. Transportation We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home if another primary care clinic is available within 30 miles. Exceptions to rides more than 30 miles from your home can be made in certain circumstances. If you need a ride and a closer clinic in our network is not available, please call our Health Ride service at or toll free. We are available daily from 8 a.m. to 8 p.m. If you are hearing impaired, please call the TTY number at or toll free or 711 for Minnesota Relay Service. UCare covers rides to a specialty clinic that are 60 miles or less. Rides to a specialty clinic farther than 60 miles from your home are generally not covered, but may be approved by UCare under certain circumstances. If you have questions, please call Health Ride. You may also be eligible for network Special Transportation Services (STS) if needed. To be eligible for STS transportation, you must have a valid Certificate of Need filled out by your physician. Under limited circumstances, UCare can approve STS rides without a Certificate of Need. If you have questions, or need help arranging a ride with a participating transportation provider, please call UCare Health Ride. Members will receive a Member Handbook with more information about transportation. Urgent/After-Hours/Emergency Care We know that you might need medical attention before your next doctor appointment. Plan members have several options available to them. Call your doctor s office for instructions on how to get urgent care or after-hours care. You can also call the UCare 24/7 nurse line for advice on getting the best treatment for your health issue. The nurse advice line is answered by nurses 24 hours a day, seven days a week. The number for the nurse advice line can be found on the back of your Member Identification Card. As a UCare member, you must call UCare Customer Services Department within 48 hours, or as soon as you are able, if you are admitted to an out-of-network hospital. In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room. Vision Care UCare pays for a range of vision services. A list of contracted providers will be in the member packet you ll receive when you enroll with UCare. You can get more information on vision care services by using the Find a Doctor feature on ucare.org or calling UCare Customer Services at the numbers listed on page i. vi
