2 Transition is helping students with disabilities and their families think about their life after high school and identify long-range goals designing the high school experience to ensure that students gain the skills and connections they need to achieve these goals the provision of funds and services to local school districts to assist in the transition process.
3 It is a federal mandate requiring youth age 16 and older to have an IEP with a postsecondary transition plan. A postsecondary transition plan must include measurable postsecondary goals for students based on age-appropriate transition assessments related to training/education, employment and, independent living skills (if appropriate) and a description of transition services including courses of study needed to assist the students in reaching their goals.
4 The following provides information on agencies and schools that provide services for students with disabilities that may assist them after high school. In addition, many of these agencies have resources to assist the student financially.
5 Arc of Washington County The Arc Washington County s mission is to enable persons of all ages with disabilities to live as contributing, valued members of the community by creating partnerships that foster nurturing communities where people with disabilities can live their vision of a valued life.
6 Hiram G. Andrews Center The Hiram G. Andrews Center provides a comprehensive program of services featuring the integration of education on campus at the Commonwealth Technical Institute (CTI), counseling, evaluation, and physical restoration in a barrier-free environment. A continuum of support services ranging from vocational evaluation and career exploration to independent living skills combines to offer our students the skills they will need to live, work and contribute in the community. The mission of the Hiram G. Andrews Center is to offer quality individualized post-secondary education, which provides career opportunities and independent life skills.
7 JobCorps Job Corps is a no-cost education and career technical training program administered by the U.S. Department of Labor that helps young people ages 16 through 24 improve the quality of their lives through career technical and academic training. The program s mission is to teach eligible young people the skills they need to become employable and independent and placing them in meaningful jobs or further education.
8 La Roche College La Roche College, a Catholic institution of higher learning, founded and sponsored by the Congregation of the Sisters of Divine Providence, fosters global citizenship and creates a community of scholars from the region, the nation and around the world. The College integrates liberal arts and professional education in creative ways.
9 Lifeswork The mission of Life swork of Western PA is to continue its comprehensive and innovative role of assisting persons with disabilities or other employment barriers to employment to achieve independence and dignity through productive employment opportunities.
10 Office of Vocational Rehabilitation (OVR) Disability Services OVR provides vocational rehabilitation services to help persons with disabilities prepare for, obtain, or maintain employment. OVR provides services to eligible individuals with disabilities, both directly and through a network of approved vendors. Services are provided on an individualized basis. The OVR counselor, during face-to-face interviews, assists customers in selecting their choice of vocational goals, services and service providers. An Individualized Plan for Employment (IPE) is developed, outlining a vocational objective, services, providers and responsibilities.
11 Community College of Allegheny County (CCAC) The Community College of Allegheny County has four campus locations (Allegheny Campus, Boyce Campus, North Campus, South Campus) and four neighborhood centers across Allegheny County. Campus life includes athletics, social organizations, honor societies, service clubs, student services, Honors Program, and more. Additionally, they offer more than 155 programs that lead to great careers and great four-year colleges.
12 PA Career Link PA Career Link is part of the commonwealth workforce development system that provides access to PA workforce development and independent living services provided by the Departments of Labor and Industry and Public Welfare.
13 Penn Commercial Penn Commercial Business/Technical School's mission is to provide the best available instruction for individual needs in the shortest time possible and at the lowest practical cost. Our training is aimed at men and women who desire to prepare themselves for careers as well as those who desire to reenter the job market. 14 Pennsylvania Institute of Health & Technology PIHT has been serving students and employers in southwestern PA for over 25 years. The school provides a small campus atmosphere that provides the following in-demand programs: Medical Assisting, Medical Office Administration, Practical Nursing, Network and Software Solutions, and Business Administration.
15 Pennsylvania Technical Institute (PTI) PTI offers students an opportunity to pursue a career in two years or less. All Associate Degree and Certificate programs offered at PTI are developed under the guidance of industry advisory boards and designed to meet the current needs of employers by following industry trends and technologies. PTI uses a hands-on approach to educate and the active learning environment replicates the reality of a workplace.
16 Project Search (UPMC/Goodwill) UPMC Project SEARCH Young Adults Program is a collaborative effort with UPMC, Goodwill of Southwestern PA, the Office of Vocational Rehabilitation (OVR), and Allegheny County Department of Human Services Office of Intellectual Disability. The program builds essential job specific and employability skills needed to gain and maintain meaningful employment, lead productive lives and become integrated in an adult work environment.
17 Southwest Training At Southwest Training Services, Inc. our mission is to help employers and job seekers make informed employment and training choices. Their staff is very knowledgeable about the local labor market, with an in-depth understanding of both sides of the employment equation (i.e., supply & demand).
18 Transitional Employment Consultants (TEC) Transitional Employment Consultants (TEC) was established in 1984 in order to assist individuals with disabilities to become productive, independent members of the community by providing job readiness training and placement into competitive employment.
19 Tri-County Patriots for Independent Living TRIPIL is a Center for Independent Living that advocates for the civil rights for people with disabilities and promotes living well with a disability in our local community. They are a membership organization of people who live in our community with disabilities and our friends. This center provides services such as peer support and attendant care.
20 Washington Communities Human Services, Inc. Washington Communities provides mental health and other supportive services to people in Washington County.
21 Triangle Technical Institute Triangle Tech takes pride in delivering highquality and hands-on career education. The students learn skills employers demand from instructors who have worked in the fields they teach.
22 Southwestern Local Task Force Mission Statement: To serve in an advisory and advocacy role to protect the rights of students with special needs; encourage cooperation among public and private agencies; accentuate the positive; and act as a catalyst enhancing collaboration in education.
23 Rosedale Technical Institute Rosedale Tech has over 60 years of training students. Founded in 1949, Rosedale Technical Institute has a rich history of preparing students for employment in the Automotive, Diesel, Electrical, and HVAC fields. Rosedale provides students with the proper knowledge and equipment, as well as the necessary hands on experience required for their field.
24 University of Pittsburgh Greensburg Founded in 1963, the University of Pittsburgh at Greensburg is a publicly assisted, four-year, liberal arts college in southwestern Pennsylvania. 30 AHEADD AHEADD is a college support program offered by the Autism Education and Research Institute (AERI) / AERI Behavioral Health Services. They assist students with progressing through college, preparing for employment, and becoming contributing, successful citizens. AHEADD coaches, mentors, and personal advocacy for college students with: Autism Spectrum Disorders, Attention Deficit Disorder (ADD), Attention Deficit Hyperactive Disorder (ADHD), Dyslexia, Learning Differences, and Non-Verbal Learning Disability (NVLD) to improve social interactions, self advocacy, organizational skills, and communication.
31 Disability Rights Network Disability Rights Network of Pennsylvania (DRN) is a statewide, non profit corporation designated as the federally mandated organization to advance and protect the civil rights of adults and children with disabilities. DRN works with people with disabilities and their families to ensure their rights to live in their communities with the services they need, to receive a full and inclusive education, to live free of discrimination, abuse and neglect, and to have control and self determination over their services. MISSION STATEMENT: The mission of the Disability Rights Network of Pennsylvania is to advance, protect, and advocate for the human, civil, and legal rights of Pennsylvanians with disabilities
32 Barber National Institute MISSION STATEMENT: We are "Making dreams come true." We provide children and adults with developmental disabilities and their families the education, support and resources needed to be self reliant, independent and valued members of their community. In addition, we provide the professionals who serve them with world-class education and training.
33 Centerville Clinics The Base Service Unit (BSU) is the "point of entry" for consumers into Mental Health/Mental Retardation programs. The BSU offers direct services and also provides referrals to numerous county and community programs. The Base Service Unit offices are located in the Yablonski Memorial Clinic and in our Greene County Mental Health Office at 190 Bonar Avenue, Waynesburg. All services offered through the Base Service Unit are confidential. No treatment and personal information is transmitted to other agencies or individuals without your written permission. Services offered include: Outpatient Services, Intensive Care Management, Student Assistance Program (SAP), and Behavioral Health Rehabilitation.
34 BrainSTEPS The Brain Injury Association of Pennsylvania, under contract from the Pennsylvania Department of Health and partnered with the Pennsylvania Department of Education, has created the Child and Adolescent Brain Injury School Reentry Program called BrainSTEPS (Strategies Teaching Educators, Parents, and Students). BrainSTEPS is working to make sure that those who must provide educational support to children with brain injury have a good understanding of brain injury, the resulting challenges, and supports and interventions that will help these students achieve educational success through graduation. The BrainSTEPS Committee is comprised of education professionals, medical rehabilitation professionals, and family members; Receives ongoing training from local and nationally recognized leaders in the field of pediatric brain injury; Provides BrainSTEPS 101 presentations, helping schools compensate for the lack of brain injury training in college teacher training programs; Provides training and consultation regarding identification, school re-entry planning, IEP development, intervention selection and implementation, long-term monitoring of students, and other issues professionals face in supporting students with brain injury; Consults with teams through the BrainSTEPS Program on a case by case basis.
