1 Slate of Candidates Elected Candidates: President-Elect, Treasurer, Central Valley/Coast Region, Northern Region and Central Region. The President- Elect serves through a three-year cycle of President-Elect, President and Immediate Past President. The Treasurer serves a one-year term. Elected directors serve two-year terms and may not serve more than two consecutive terms in the same position. President Elect (1 candidate) Kevin Russell, RPh Kevin grew up in Corvallis, Oregon and graduated from the OSU College of Pharmacy in While at OSU, Kevin served as President of the Student Executive Council and as the ASP/APHA NW Regional Delegate. Kevin has worked as a staff/clinical outpatient pharmacist for Kaiser Permanente, co-owner of Philomath Pharmacy and as a pharmacy manager for Safeway. Kevin s current position is District Pharmacy Manager for Safeway. Kevin has served his community as a member of the Oregon Drug Utilization Review Board (2 years), Cubmaster/Scoutmaster Boy Scouts of America (8 years), OSPA Convention Planning Committee (4 years) and OSPA Board of Directors (2 years). Kevin just completed his MBA from OSU in Executive Leadership. Kevin resides with his wife and 3 boys in Albany, Oregon. OSPA and the profession of pharmacy in Oregon have achieved many victories this year. Provider status, expanded drug administration and pharmacist prescribed birth control will provide many new opportunities for our profession. Leadership by our association is more important than ever. As President-Elect and President, I will support our continued mission of professional education and legislative advocacy. I will also work towards further integrating pharmacy and pharmacists into the health care systems of our communities. I will focus on creating turnkey solutions for community pharmacists to participate in and be compensated for collaborative care to improve the health outcomes of patients in Oregon. This is also a time for unification-pharmacists collaborating and speaking with one voice. Retail pharmacy, ambulatory care, long-term care and specialty pharmacy are all part of an increasingly connected health system. OSPA can be the link that brings everyone together to strengthen our profession and our abilities to serve our patients. The future of pharmacy we have been advocating for is here. It is now time to leverage our opportunities to achieve a new role in healthcare. It will be an honor to lead and to serve OSPA and the pharmacists of Oregon as President.
2 Treasurer (1 candidate) Linh Chau, RPh Linh Chau graduated in 2001 from Oregon State University with a Bachelor s of Science in Pharmacy. Shortly after graduation, he worked at Northwest Homecare/OHSU, LLC helping patients transition from the hospital to the comforts of their own home. Most of his 14- year pharmacy career has been in Community Practice where he finds his passion. He finds the opportunity to engage and educate patients to be the most rewarding aspects of a pharmacist s job. He has been a pharmacy manager for five years and district manager for seven years. Outside of work, he enjoys spending time with his beautiful wife and two younger daughters. He also finds peace struggling with golf and working on multiple home projects at once. It is a very exciting time in our profession. We can either sit back and let these changes shape us or be at the forefront and shape it into what we want. I hope to help fuel this momentum to forever change and evolve the clinical practice of pharmacy. Like my previous leadership experiences, I see this as a tremendous opportunity to change how pharmacy practice is defined. What interests me most about leadership is the opportunity to lead and teach our next generation of pharmacist on the fine balance between pharmacy operations and clinical care. In addition to being a pharmacist, I bring a unique array of business experience from the many small businesses that I have owned and operated. My leadership style is best described by Ernest Hemingway, "Give a man a fish, feed him for a day. Teach a man to fish, feed him for a lifetime." Thank you for your consideration. Central Valley/Coast Region Delegate (1 candidate) Tiffanie Pye, PharmD, BCPS Tiffanie Pye is a 2011 graduate of Oregon State University (OSU) College of Pharmacy and Oregon Health & Science University (OHSU). She is currently working as a community and consultant pharmacist in Stayton, Oregon. She is the pharmacist in charge of Stayton Pharmacy and splits her time with Sublimity Pharmacy as a LTC consultant. She has developed an MTM program for Stayton Pharmacy and led the pharmacy to set up a successful synchronization model called Healthminder. She implemented a workflow system to increase patient safety and utilize staff more effectively. She was awarded a grant from Marion County Health Department to help increase vaccinations and provided over 100 influenza shots to uninsured or underinsured patients. Sublimity Pharmacy contracted with a local hospice and Tiffanie has worked to implement policies and procedures as well as clinical oversight to meet the needs of this unique population. She is a current preceptor and has mentored several students. Tiffanie volunteers at the OSU College of Pharmacy as a member of the Assessment Committee and as a Young Alumni, sharing stories and successes with new and graduating students. Tiffanie earned her Board Certification in Pharmacotherapy in I am honored to be nominated to the Board of Oregon State Pharmacy Association. I am excited for the opportunity to represent my fellow pharmacists on this board. I am proud of our profession and I want to be on the forefront of its growth. I believe I am well suited to serve on this board. Thank you for your consideration.
3 Northern Region Delegate (1 candidate) Allen Ha, PharmD Allen Ha, Pharm. D. is currently a Pharmacy Manager for Fred Meyer in the greater Portland area. He completed his pharmacy education at Oregon State University (OSU) College of Pharmacy and Oregon Health & Science University (OHSU) in While in school, Allen held a variety of leadership positions which included APhA Counseling Competition Coordinator, Oregon State College of Pharmacy Executive Council member, P3 Class Representative and P4 Class Representative. His excellence in academia and dedication as a student ambassador was recognized with multiple awards including P2 Unsung Hero, Fred Meyer Corporation Scholarship, NACDS National Foundation Pharmacy Student Scholarship, OSSP Member of the Month (February 2011), APhA-ASP Leadership Certification Award, McKesson Foundation Scholarship, Oregon State College of Pharmacy Class of 2012 Most Outstanding Student Award and induction in the Phi Lambda Sigma Pharmacy Fraternity. Allen initiated his pharmacy aspiration by working as a pharmacy technician with Fred Meyers eight years ago and then later applied to the PharmD program at Oregon State University/Oregon Health & Sciences University. He accelerated his passion for the field of pharmacy through an internship with Fred Meyer and was recently promoted to Pharmacy Manager after working as a Staff Pharmacist for two years. Allen took upon the challenge of managing a low performing store and turned it into one of the top three stores within the district in six months. His management style and ability to drive results have been recognized by his staff, peers and senior leaders at Fred Meyer. Allen continues to stay active in the pharmacy community by volunteering as a preceptor at numerous health care events including the Chinese New Year Festival, Vietnamese Tet Festival, Asian Community Health Fair and the Diabetes Expo. Upon graduation, Allen recognized that there was a need in the Asian community for non-english speaking patients to gain visibility for diabetes and blood pressure testing. One of the key elements was staffing the community events with P3 and P4 students that were fluent in the native languages of the community members so that it would draw in individuals that may not typically seek medical attention due to a language barrier. His partnership with Oregon State University and Fred Meyer also allowed him to offer these resources and services free of charge to members of the community. Additionally, he maintains his support to the OSU College of Pharmacy as the Oregon State College of Pharmacy Class of 2012 Alumni Representative. I am proud to be part of a progressive state where the field of pharmacy is continuously improving and now recognizing pharmacists as health care providers. Oregon State Pharmacy Association has the capacity to shape the development and progression in pharmacy that will impact our pharmacist roles. Having an innovative professional organization combined with a strong leadership team will allow the field of pharmacy to advance as health care providers who will be at the forefront of pioneering a dynamic and effective pharmacy foundation. My dedication to advocate for the members of OSPA and to positively impact the overall healthcare system in Oregon will make me an excellent candidate for the Northern Region Delegate position. Thank you for your consideration as I will do my best to serve on the OSPA Board of Directions.
4 Central Region Delegate (1 candidate / 2 nd Term) Robert Uetrecht, RPh Robert Uetrecht is a Central Oregon pharmacist with over 20 years experience. Rob s pharmacy career started at the age of 16 in a small Indiana pharmacy. He has worked in numerous settings including hospital, home infusion and retail. He currently runs a small successful pharmacy in Bend (Pharmacy Express). In growing that business he has established himself as a highly respected professional in the community with his patients and the medical profession. Rob has accomplished many things in his career: starting a smoking cessation program, growing a retail pharmacy to successful profitability, choosing a new computer system company wide, assisting with the implementation of said system and spearheading MTM within his company. He excels at counseling his patients and assisting them with their health care. His passion for teaching patients drives his success. You will often find Rob riding and racing his bikes with his family and friends. He uses those experiences outside of his job to grow himself as a profession. His competitive nature shows in his racing and his drive to be the most successful he can be. I wish to apply for the current open OSPA board position so that I may contribute what I have learned while working in the Central Oregon community for the past 17 years. I see pharmacy approaching a pivotal point. Patients, more than ever, need pharmacists to help guide them with their health care. I believe that my experience will be valuable in shaping how pharmacy will assist them. I wish to be a part of that process. * * * * * * * Appointed Candidates: Two (2) Board Members at Large, two (2) faculty representatives one (1) from each College or School of Pharmacy in Oregon, two (2) student representatives - one (1) from each College or School of Pharmacy in Oregon and one (1) technician member. Appointed directors serve two-year terms and may not serve more than two consecutive terms in the same position. Board Members at Large: One (1) Pharmacist member at large may be appointed from each of the following practice settings: Community practice (independent, chain, grocery, or mass merchandise) Consultant and Long-Term Care Health-System/hospital/institutional Directors for these positions may be nominated by an appropriate practitioner organization (e.g., OSHP, OSCP) with approval by affirmative vote of two-thirds (2/3) majority of the Board of Directors. When making appointments, consideration shall be given to having a balanced representation on the Board of managerial (owners, managers, directors) and staff practitioners.
5 Board Member at Large Consultant/Long-Term Care (1 candidate) Charles Huckabay, PharmD Charles Huckabay is known among his colleagues as a natural leader, hard worker and problem solver. He is a graduate of University of Washington School of Pharmacy It was at UW that he focused his clinical rotations in a variety of settings such as kidney transplant, oncology, long-term care and pharmacy management. His professional career is based in multiple settings, including retail and long-term care with additional experience in travel medicine. He was the Assistant Director of Pharmacy operations for Signature Pharmacy Solutions, and currently holds a position as a Consultant pharmacist for Consonus Pharmacy. He has a passion for geriatrics and currently is an advisor for Oregon Care Partners (OCP) where he has been helping create a curriculum to help educate the public on Understanding Medicines in Older Adults, thanks to the funding provided by the State of Oregon and OCP partnership with Oregon Health Care Association. Pharmacy is an ever expanding, complex and dynamic industry. Changes to healthcare are occurring rapidly and will impact all healthcare professions. With these changes in healthcare come the need for leadership and positive pharmacy voices. I believe that OSPA has a tremendous opportunity to contribute to the future of pharmacy, and I value the opportunity to serve if elected.
