2 Financial Services and Credit Guide Version 1.1 Issue date 28 May 2015 Important information We recommend you read this Financial Services and Credit Guide (FSCG) because it contains important information designed to help you decide whether to use any of the financial services offered by us, as described in this Guide, including: who we are and how we can be contacted information about AMP Financial Planning, our Licensee the initial and ongoing advice and services we provide our fees and how we are paid in connection with those services your privacy how we deal with complaints This FSCG comprises two parts and is only complete when the business profile and adviser profile are together.

3 AMP Spark Advice A successful life means different things to different people. But whatever path you re on, we want to help you find a balance between three key areas: Wellbeing, Prosperity, and Legacy. Attached to this document are your advisers profiles. The individual profiles contain information about your advisers; their contact details, qualifications, experience and any memberships they may hold. It also outlines the types of advice, strategies and products about which your adviser can provide advice. Our financial advice process Our market leading financial advice process understands everyone is different, with different circumstances, needs and goals. We treat every client as an individual, and our unique advice process allows us to tailor a plan that best meets your goals. A copy of the process is available after the first meeting and through your online dashboard. Documents you may receive We will provide you a number of documents as you progress through our financial planning process, to keep things simple and to capture each stage of your advice journey. At the beginning, we [one of our life coaches will] explore your goals with you. We [Your coach] may provide you with general advice at this stage, that isn t tailored to your individual circumstances, to help explain general concepts and options relating to the strategies and products we advise on. At the end of this conversation, we will provide you with a Goals Summary document. When we provide personal advice which is tailored to your personal circumstances, it will normally be documented and provided to you in a Statement of Advice (SoA), known as a financial plan. The financial plan contains a summary of your goals and the strategies and any financial products we may recommend to achieve your goals. It also provides you with detailed information about product costs and the fees and other benefits we will receive as a result of the advice we have provided. If we provide further personal advice, a financial plan may not be required. We will keep a record of any further personal advice we provide you for seven years. You may request a copy of such records by contacting our office during that period. When we provide credit advice, we will conduct a preliminary assessment to determine the suitability of a particular product. This is normally documented and provided to you in a SoA, a record of debt advice, or a Credit proposal. We will retain a record of the debt advice which you may request by contacting our office within seven years of the assessment. If we recommend or arrange a financial product for you, we will provide a product disclosure statement (PDS) or investor directed portfolio service (IDPS) guide where relevant. These documents contain the key features of the recommended product, such as its benefits and risks as well as the costs you will pay the product provider to professionally manage your investment or insurance.

4 You should read any warnings contained in your SoA/financial plan, record of debt advice, Credit Proposal, the PDS or IDPS guide carefully before making any decision relating to a financial strategy or product. The cost of our advice Goals exploration We charge $150 (incl GST) for a one hour consultation to help you determine the goals that are most important to you and the supporting advice strategies that may be required to bring them to life. This is a goals based coaching conversation that prioritises what s important to you and the potential scope of any further advice to meet your personal circumstances and objectives. At the end of the goals meeting you will receive a Goals Summary documenting the priority of the goals you would like to achieve, and the costs to receive advice that will take into account your personal circumstances and needs. If you choose to proceed to receive personal advice, the cost of the goals consultation is deducted from our advice fee. A personalised plan The initial fee covers: 1. the cost of researching, preparing and implementing your personal financial plan based on your goals and any relevant strategies; and 2. any on-going service arrangements in the first year post implementation. If you proceed we will explain clearly which of the plans below is most appropriate for your circumstances, and therefore which fee will apply. These fees include GST. Basic Advance Premier $900 $2,750 $9,000 Sticking to your plan The ongoing advice agreement, which is provided at the same time as your SoA, sets out our support packages, including the advice and services we will provide, as well as the frequency these will be delivered, how much it will cost, your payment method and how the service can be terminated. These fees include GST. Assist Engage Partner $330 $1,500 $4,500 Our fees reflect the cost of the work we do for you in bringing your goals to life. We therefore expect from time to time that it will be necessary for us to amend our fees. As a general guide, we will consider our fees on an annual basis and make any necessary changes on or around 1 July each year. If we change our fees we will let you know and you can terminate our services at any time. Additional advice For all other advice, an additional advice fee of $330 per hour (incl GST) may be charged. Any additional advice fee will be agreed with you before the advice is prepared and will be disclosed in your SoA.

6 Transaction services If you do not require advice, but you would like for us to help you complete a particular transaction, we can arrange for you to apply for the kinds of products referred to in this guide. In these cases, we can take your instructions and arrange for the transaction to be completed without providing you with personal advice. You can also contact us directly with any instructions relating to your existing financial products. We will accept instructions from our existing clients via telephone, mail and (where we have authority to do so). Payment methods We offer you the following payment options for payment of our advice fees: Bpay, direct debit (credit card or savings) and cheque Deduction from your investment or commission payments that be paid from some products Other payments Insurance Please note that for services in relation to insurance, we receive payments which are factored into the annual premium and are used to cover the cost of your advice fees. These may range from 0% to130% of the first year s insurance premium paid, and/or An ongoing payment of between 0% and 33% per annum of the following years premium paid to us by insurance providers. Banking Arrangements You may be referred to AMP Bank for home and business lending services. We receive an initial payment of up to 0.65% and an ongoing payment of up to 0.25% of the loan amount. For example, for a loan of $100,000 we would receive an initial payment of up to $650 and up to $250 for each year the loan is in place

7 Payments to and from other professionals We may receive payments to refer you to other service providers. These amounts do not involve additional costs and will be disclosed in your statement of advice. Where you have been referred to us by someone else we may pay them a fee, commission or some other benefit in relation to that referral. Our current referral arrangements are detailed below: Referrals to: Service provider Referral service and payment arrangement Paid to Nature of relationship Stockbroker approved by AMP Financial Planning Advice and/ or execution of share/securities transactions 2.5% of any referral fees will be paid to AMP Financial Planning and we will receive the remaining 97.50%. For example, if the referral fee was $1,000 AMP Financial Planning will receive $25.00 and we will receive $ AMP Financial Planning and Our practice No relationship Legal services Carden & Co. Pty Ltd We offer a facilitation service for legal services with Carden & Co. Under this service, we arrange for clients to meet with a solicitor from Carden & Co. for legal services, particularly estate planning advice. Under this service, Milestone facilitates the service including the billing of fees. Milestone retains 33.3% of the fees charged and passes on 67.7% to Carden & Co. AMP Financial Planning and our practice No relationship For example, if the fees charged for estate planning advice were $1,000, Milestone would retain $ and Carden & Co would receive $ Property services Australian Property Masters Pty Ltd We have a referral arrangement with Australian Property Masters through which we refer clients for direct property advice. If you are referred by us, then we will receive a referral fee of 75% of the sales commission received by Australian Property Masters which is usually 2.2% of the property value. For example, if Australian Property Masters received a sales commission of $8,800, we would receive a referral fee of $6,600. AMP Financial Planning and our practice No relationship Referrals from: Referrer Payment arrangement Paid to The Income Tax Professionals (ACT) We pay The Income Tax Professionals a referral fee of 12% of any fees and/or commissions received by us. For example, if we charge you a fee of $2,000, we would pay The Income Tax Professionals a referral fee of $240. The Income Tax Professionals (ACT) No relationship Distribution of fees Your fees and if applicable, additional payments paid by product providers (total payments) are receivable by AMP Financial Planning Limited as the Licensee and distributed to Milestone Advice. Further distributions to your adviser will be explained in their adviser profile.

8 Approved products and services AMPFP maintains a broad approved product list, containing investment and insurance products, and direct investments that have been researched by external research houses as well as our in-house research team. It includes products issued by AMP companies and a diversified selection of approved Australian and International fund managers. AMPFP periodically reviews the products and direct investments on the approved list to ensure that they remain competitive with similar products that could address your needs and objectives. A copy of the approved product list can be supplied to you upon request. If we recommend a new platform or portfolio administration service, we use those issued or promoted by the AMP Group or as otherwise approved by AMPFP and where appropriate to your circumstances. As at July 2014, the lenders and lessors whose products are most commonly recommended by Accredited Mortgage Consultants authorised by AMPFP are: Lenders AMP Bank ANZ Homeside Lending Commonwealth Bank Suncorp St George Westpac Lessors Macquarie Esanda Westpac Commonwealth Bank Relationships and associations It is important that you understand the relationships that AMPFP has with other providers of financial services and products as they could be seen to influence the advice you receive. AMP Limited is listed on the Australian Stock Exchange. Advice we can provide Milestone Advice can provide you with both personal and general advice about strategies and products listed below. We can also arrange for those financial products to be issued without advice from us. General advice means we have not taken into account your circumstances, financial situation and needs when we provide financial product advice. Personal advice means we have taken into consideration your circumstances, financial situation and needs when we provide financial product and service advice. Individual advisers within Milestone Advice may not be qualified to provide advice in all of the strategies and products noted below. Their individual profile guides will note the advice they are qualified to provide. At all times Milestone Advice will ensure the appropriate adviser is available to you to provide advice consistent with your goals.

12 Other benefits or Support we may receive To provide our new advice experience to you, Milestone Advice is entitled to a supplementary payment from AMPFP. Until the end of 2015, the supplementary payment will consist of: up to 40% of any advice fees that you agree to pay us in respect of Basic and Advanced advice package; and 20% of any on-going service fees you agree to pay for the same advice packages. This Payment is paid by AMPFP and is completely independent of any product or service that we may recommend and is at no cost to you. In 2016 these payments will reduce to 20% and 10% respectively and will cease from 2017 onwards. We may be offered or receive non-monetary benefits such as attendance at training events, entertainment or sponsorship from some product providers at no extra cost to you. Both AMPFP and Milestone maintain a register to record the non-monetary benefits that we receive. A copy of this register can be made available to you within one month of your request. Business buy-back option: If we leave the financial services industry or can no longer appropriately service a selection of our clients due to retirement, permanent disability or death, AMPFP will either look after our clients or appoint one of its authorised representatives to do so. If this happens, AMPFP may buy back our business. The amount will vary depending on a number of factors including, our reason for leaving the financial services industry, the time our business has been established, the annual recurring revenue (both actual and any deemed revenue) of our business and the quality of our previous advice. Your privacy Another financial adviser may be appointed to you if your adviser leaves AMPFP or is unable to attend to your needs due to an extended absence from the business. In these circumstances, AMPFP will write to you advising you of the change. Your personal information will be passed on to the new adviser. If you choose to appoint a new financial adviser, your new adviser will be provided access to your policy information. They will be responsible for providing you with ongoing advice relating to those policies and all future advice fees deducted from the policy/(ies) will be paid to your new adviser.

13 Privacy Collection Statement As part of the financial planning process, we need to collect information about you. Where possible we will obtain that information directly from you, but if authorised by you we may also obtain it from other sources such as your employer or accountant. If that information is incomplete or inaccurate, this could affect our ability to fully or properly analyse your needs, objectives and financial situation, so our recommendations may not be completely appropriate or suitable for you. We are also required under the Anti-Money-Laundering and Counter-Terrorism Financing Act (AML/CTF) 2006 to implement client identification processes. We will need you to present identification documents such as passports and driver s licences in order to meet our obligations. We keep your personal information confidential, and only use it in accordance with our Privacy Policy. Some of the ways we may use this information are set out below: Your adviser and AMPFP may have access to this information when providing financial advice or services to you; Your adviser may, in the future, disclose information to other financial advisers, brokers and those who are authorised by AMPFP to review customers' needs and circumstances from time to time, including other companies within the AMP group; Your information may be disclosed to external service suppliers both here and overseas who supply administrative, financial or other services to assist your adviser and the AMP group in providing financial advice and services to you. A list of countries where these service providers are located can be accessed via the AMP Privacy Policy from March Your information may be used to provide ongoing information about opportunities that may be useful or relevant to your financial needs through direct marketing (subject to your ability to opt-out as set out in the AMP Privacy Policy); Your information may be disclosed as required or authorised by law and to anyone authorised by you. Your adviser and AMPFP will continue to take reasonable steps to protect your information from misuse, loss, and unauthorised access, modification or improper disclosure. You can request access to the information your planner or AMPFP holds about you at any time to correct or update it as set out in the AMP Privacy Policy. Professional indemnity insurance Professional indemnity insurance is maintained by Milestone Advice to cover advice, actions and recommendations that have been authorised by AMPFP and provided by your adviser. The insurance satisfies the requirements imposed by the Corporations Act 2001 and financial services regulations. Confidence in the quality of our advice

14 If, at any time, you are not satisfied with the advice you receive, or any other aspect of the service provided by your wealth adviser, you should take the following steps: Contact your adviser and tell them about your complaint.
