1 Service Accountability Agreement Diabetes Education Programs Central East LHIN Board of Directors May 22, 2013

2 History The Diabetes Regional Coordination Centre (RCC) Program was approved by the Ontario government in October 2009 to support the goals of the Ontario Diabetes Strategy The Ministry established 14 RCC sites across the province to work collaboratively with the LHINs and health service providers to address regional priorities related to diabetes care Effective April 1, 2013, the operational mandate, functions and funding for delivery of the RCC program were transferred to the LHINs 2

3 Purpose Diabetes Education Programs (DEP) are to provide education and self-management support for people with diagnosed prediabetes, Type I-Insulin Dependent, Type II-Non-Insulin Dependent and Gestational Diabetes Target of 1000 clients per year per team (1 Registered Nurse/1 Registered Dietitian) A portion of their work will involve outreach into the community, delivering community events that are designed to deliver several ways to educate and inform the community on diabetes prevention and management, as well as connect them with other resources both in the community and beyond 3

4 Work Plan/Deliverables The LHIN activities within the Diabetes/Vascular Health program will focus and contribute towards improving the following 5 system attributes/goals as they relate to diabetes services, Diabetes Education Programs (DEP) and clinical services as; 1. Equitable 2. Accessible 3. Patient-centered 4. Effective 5. Integrated 4

5 Current State (1) 12 Diabetes Education Program (DEP) have been transferred to Central East LHIN Community Health Centres; o Brock Community Health Centre o Oshawa Community Health Centre o Port Hope Community Health Centre o Scarborough Centre for Healthy Communities o TAIBU Community Health Centre Hospitals; o Campbellford Memorial Hospital o Haliburton Highlands Health Services Corporation o Lakeridge Health o Peterborough Regional Health Centre (Pediatric and Adult) o Ross Memorial Hospital o Scarborough Hospital o Rouge Valley Health System (Pediatric and Adult) 5

6 Current State (2) All of the Diabetes Education Programs that have been transferred have existing Multi-sectoral Service Accountability Agreements (M-SAA) with the Central East LHINs Required to issue an amendment to Schedule E3 (LHIN Local Indicators) to accommodate the Diabetes Education Program reporting group and execute amended M-SAA agreements by June 30,

7 Current Reporting Requirements On a quarterly basis, the health service providers with Diabetes Education Programs funded by the LHIN are required to provide an update to the Diabetes and Vascular Health Team Information provided will reflect its financial statements and program reports - Aligns with Schedule B1-Financial 7

8 Provincial Working Group Working group to be established to develop provincial indicators by October 2013 with representation by each 14 LHIN Leads Purpose Collaboration among all LHINs to identify a standardized and common set of measureable indicators for the core activities, with a reporting/assessment plan that measures the impact of the program The LHIN will be required to provide regional results to be included in an annual program report to the Ministry. 8

9 Motion Be it resolved that the Central East LHIN Board authorize the Board Chair and LHIN CEO to execute amended Multi-sectoral Service Accountability Agreements to incorporate the Diabetes Education Programs by June 30, 2013 with the following agencies; Community Health Centres; o Brock Community Health Centre o Oshawa Community Health Centre o Port Hope Community Health Centre o Scarborough Centre for Healthy Communities o TAIBU Community Health Centre Hospitals; o Campbellford Memorial Hospital o Haliburton Highlands Health Services Corporation o Lakeridge Health o Peterborough Regional Health Centre (Pediatric and Adult) o Ross Memorial Hospital o Scarborough Hospital o Rouge Valley Health System (Pediatric and Adult) 9
