5 Welcome to Patient Choice Insights by Medica! As a health care consumer, you have choices to make. Medica offers this Provider Directory as a tool to help you get the most from your plan. To provide more affordable choices, your Patient Choice Insights by Medica plan offers you a wide range of choices for providers and hospitals. To help you better understand the wide variations that exist in the cost and quality of health care, providers are placed into three tiers, based on cost and quality measures. The lower the tier, the better they have performed on the measures. When you receive services from providers in a lower tier, your share of the cost of your care is generally lower. It is also important for you to know that going to health care providers or hospitals outside of your network will result in higher costs for you, often significantly higher. Where to get the most current information. The provider tier information in this directory is effective January 1, 2014, through December 31, Changes can occur in the network and may not be reflected in this directory. To get the most up-to-date look at who is in the Patient Choice Insights by Medica network, please use the online search tool at mymedica.com > Find Physician or Facility. The online tool is updated weekly and allows you to search for Patient Choice Insights by Medica health care providers by location, name, specialty and tier. Get help finding a network provider that meets your needs. If you re unable to find a provider using this directory or the online tool, call Customer Service using the phone numbers at the bottom of this page. Call Monday through Friday, 7 a.m. to 8 p.m. (closed 8 9 a.m. Thursdays) and Saturdays, 9 a.m. to 3 p.m. Customer Service can also provide this information in Braille or large print. Facts about the Patient Choice Insights by Medica network. Providers and facilities in the Patient Choice Insights by Medica network have been placed into three tiers. The tiers will help you identify those highquality providers who are more efficient and cost-effective. The tiers will also represent the amount you pay as a copayment. Please refer to your Summary of Benefits for further information. Tier 1 ($) = lower cost Tier 2 ($$) = middle cost Tier 3 ($$$) = higher cost It s your choice. You may go to any provider in the Patient Choice Insights by Medica network primary care physicians, specialists, hospitals, and other health care professionals and facilities in any tier, without a referral. The costs you pay your copayment may vary depending on the provider you choose. When you visit providers who have performed better on cost and Buzzwords Network: A group of doctors, hospitals and other health care providers contracted to provide services to Medica members for less than their usual fees. Members typically pay less for using a network provider. Provider: A doctor, clinic, hospital, pharmacy or other health care professional or facility that is licensed, certified or otherwise qualified under state law to provide health care services. Tier [1 ($), 2 ($$) or 3 ($$$)]: Used to identify how providers in the Patient Choice Insights by Medica network compare overall on a variety of cost and quality measures. The lower the tier, the better a provider has performed on the measures. 6 Patient Choice Insights by Medica Provider Directory Buzzwords Copay: A fixed amount of money you may be required to pay each time you see a provider. Summary of Benefits: The legal document (policy) that describes your coverage in detail. quality measures, for example providers in Tier 1 or Tier 2, the copayment and/or percentage of coinsurance you pay are usually lower. Tiers are assigned at the clinic/facility level. A doctor s tier information can vary by practice location. Be sure to verify your doctor s tier for the address at which you plan to receive services. Some services are non-tiered. For example, if the plan offered by your employer covers preventive services at 100 percent, you receive the same level of benefits, regardless of whether the provider you choose for these services is in Tier 1, Tier 2 or Tier 3. You must use a network hospital to ensure receiving your highest level of benefits. Sometimes even network doctors are not aware that they need to send you to a network hospital for inpatient care. Work closely with your doctor to choose the most appropriate facility for your needs within the Patient Choice Insights by Medica network. Otherwise, your costs will be much higher and you may be responsible for paying the entire cost of the services you receive. If you need to go to a hospital, use the Hospital Comparison tool available online at mymedica.com. You can use this tool to compare hospital performance on a variety of measures such as their experience with treating certain conditions, patient safety, patient satisfaction and more. The Patient Choice Insights by Medica network is always being updated. Because providers are added to or drop out of the Patient Choice Insights by Medica network, this printed edition of the directory may not include the most current information. Also, providers may change tiers from year to year. The new provider tier information becomes effective on January 1 and will be available online for you to review during the fourth quarter of the current year. Prior to getting care, take a minute to verify that your provider is still in the network and which tier applies to that provider. A provider in a lower tier may not always be the best choice for you. When you get health care services, you are making a purchasing decision. And just as you consider many factors when making other purchasing decisions, the lowest priced option may not best meet your health care needs. Considerations such as special expertise, experience with your condition and convenience are just a few examples of other factors to weigh in your decision-making process. Tier information should not be your only consideration when choosing a provider. The information about provider performance should only serve as a guide and is not a guarantee of quality service or treatment outcome. You should know that there are many ways to measure provider performance and any process has a chance of error. It is important that you consult as many other sources as possible, including your doctor, to help make informed choices about your care. How to use this Provider Directory Use this directory to find a provider that is conveniently located for you and can meet your needs. Inside, providers are arranged: (1) by specialty, (2) by state and city, (3) and then, by clinic in alphabetical order beneath each specialty. Tier information is listed by each clinic/facility. To find a provider: 1. Choose a specialty. 2. Choose a city where it would be convenient for you to see a provider. 3. Choose a clinic under that specialty. Each listing includes the clinic address, phone number, tier assignment and a list of the physicians who practice at that clinic. 4. Consider the tier of your provider selection and how that may affect your out-of-pocket costs. Member Tip: If you already know the name of the provider you would like to see, use the index at the back of the book. It lists providers in the Patient Choice Insights by Medica network alphabetically, by name. Patient Choice Insights by Medica Provider Directory Sometimes, a clinic in the network may choose not to accept new patients. However, there are special reasons that you could still go to that particular clinic, such as: (1) You are already a patient at the clinic, but are changing health plans; (2) You are already signed up at the clinic, but have not been seen yet; (3) You are an immediate family member of someone who is a patient at the clinic. To find out if a clinic will accept you as a new patient, contact the clinic directly. Enrolling in this health plan does not guarantee you can see a particular provider on this list. If you want to make sure you can receive care from a specific provider, you should contact Medica Customer Service to ask whether that provider is still a Patient Choice Insights by Medica provider (refer to contact information at the bottom of this page). You should also contact the provider s office to find out whether the provider is still accepting patients. 8 Patient Choice Insights by Medica Provider Directory Buzzwords Primary Care Physician: A doctor you choose from within your care system who will provide preventive and other health care services. Primary care physicians generally specialize in family or general practice, internal medicine, pediatrics or obstetrics/ gynecology. Specialist: A provider whose medical specialty is not in the field of family or general practice, internal medicine or pediatrics such as a cardiologist or dermatologist. Tips for choosing a provider. Medica encourages you to establish a long-term relationship with a physician you can trust. A doctor who knows you and your needs can be one of your most valuable health resources. These types of physicians are often referred to as primary care physicians. The most common types of primary care physicians include: Family practice physicians. These doctors specialize in taking care of the general health needs of people of all ages, from infancy through adulthood. They receive a broad range of training in: pediatrics, internal medicine, obstetrics and gynecology, psychiatry and geriatrics. Internal medicine physicians. These doctors specialize in diagnosing and treating diseases in adults. They do not perform surgery, deliver babies or treat children. Pediatricians. These doctors specialize in taking care of the general health needs of children, from birth through approximately age 16. Obstetricians/Gynecologists (OB/GYN). These doctors specialize in pregnancy and childbirth and in the diseases and routine care of the reproductive system of women. It is important to choose the right physician to provide for the health care needs of you and your family. When choosing a doctor, you may want to consider: Training and experience. Find out whether the doctor is board-certified and whether they have any special training. You can check a doctor s qualifications by using the resources below. Accessibility. You want your doctor to be available when needed. Ask about office hours, how long it takes to schedule a visit and how patients afterhours needs are handled. Some providers offer extended hours. Contact your clinic for more information. The doctor s approach to preventive medicine. You want a doctor who emphasizes staying healthy. A good indicator of this is if the doctor asks about your eating habits, exercise routine, alcohol or tobacco use or any other health risk factors. Reassure yourself: Check out a doctor s qualifications.
