2 Introducing Broker Application Before completing the following form it is essential that you have read and understood all the terms and conditions concerned with your Next trading account as detailed in the Terms of Business Document. Please complete all sections of this from. When completed, this application will form part of your Customer Agreement with Next. We will use the information gained from this application form to assess your suitability to open an account. For this reason it is essential that all the information provided is entirely accurate and if, at a future date, any circumstances adversely affect this information you are required to write to us advising us of these details. Copy of Register of Share Holders a. Individual verification of share holders with a holding of 25% or more b. Individual share holders (copy of passport and a recent residential utility bill for each) c. Corporate share holders (a complete list of supporting documentation for each company as listed above) We may need to contact you for further information/documentation Overseas Clients resident in certain jurisdictions might be required to provide two certified forms of address verification + one certified proof of identity. Those certifying documents must sign the documents stating: I certify this to be a true copy of the original and provide their full contact details such as name, phone number and address. Certification may be carried out by a lawyer, tary Public, consulate or embassy official and be stamped as true copy of the original. All copies must be clear and certification must be legible, and signed with the full name and address of the certifier included.
